Employee had connected the miner tail with a chain to the front bumper of a shuttle car to attempt to free the car from being stuck. Once the chain became tight the chain came loose contacting the employee in the lower back resulting in a fractured vertebra.
Employee was in the process of installing a roof bolt when EE used the rotation to align the chuck and the bolt head causing EE's gloved hand to be strained.
Employee was backing the bolter out of the #7 entry when the cable came off the side and ran under the cab. Employee reached from the deck behind the bolter to adjust the cable while backing up and contacted a de-energized car with the back of the bolter catching their left arm causing a laceration/ avulsion that was surgical repaired.
The employee smashed the tip of right small finger between the outside of the boom and the inside of the valve assemble located on the operators compartment while installing chain hangers.
Employee was drilling the roof when a piece of slate broke loose and came down the drill steel revealing a small nodule that also came loose from the roof and contacted the top of the employees left boot. No fractures found and the employee was treated for a bruise to EE's left foot.
Employee was using a bolter wrench to tap the dust out of EE's drill steel. After striking the drill steel the employee struck self with the wrench in the mouth. It was determined on 8/11/25 that EE had a chipped tooth that would need repaired.
Employee was installing wire mesh to the mine roof during the bolting process when a piece of slate fell between the mesh contacting the employees gloved hand that was resting on the tray resulting in a laceration that required stitches.
Employee was operating a roof bolter on #3 unit, while on EE's knees with EE's lower leg behind EE a piece of slate struck EE's left foot twisting it causing a sprain/strain. The employee finished EE's shift but was taken off work for EE's sprain the next day.
Employee was in the process of moving the roof bolter when cable slack was picked up by the bolter operator engaging the reel, causing the cable to contact the injured employee in the mouth, resulting in sutures to employee's bottom lip.
Employee was operating a roof bolter when a nodule (2"x 8"x 4") that was unknown to the operator to be resting on the canopy slid off and struck the employee in the right hand resulting in a laceration that required sutures.

The employee was attempting to straighten the roof bolt in the #9 entry on the #5 unit when the bolt spun causing EE to strain left shoulder. EE is being treated for a strained left shoulder.
While the employee was trimming the roof with the miner in 8 right a piece of metal struck EE in the upper left bicep that required 2 stitches on the initial visit. The metal was surgical removed on 12/08/2023.
While advancing the bolter to the next row of bolts to be installed on #3 unit the employees left foot was ran over by the rear tire of the bolter resulting in a bruised foot. The employee wasn't able to return on 10/17/23 because of the bruising.
The employee was swinging the boom of the roof bolter in when a piece of coal rib broke loose striking the employee in the upper back. The employee received a few scratches to their back and some soreness in their right rib area.
Employee was struck by a rock causing a laceration that required stitches.
Employee was cutting a wear plate off of a continuous miner. As EE was cutting, the torch hit some dirt causing some slag to go into EE's eye.
Employee had pinner steel resting on inside of leg when employee dropped the boom to add second steel. Boom hit tip of steel pressing into employe's upper right thigh. Deep bruise to upper right thigh
Employee was tramming the roof bolter, when a piece of the mine floor came out from under the bolter tire and rolled the employee's right ankle. Employee has a sprained ankle.
Employee put EE's pinner steel in EE's tray after putting a pin up. When the pinner steel got caught by the boom on the way down and it hit employee in the mouth and chipped EE's teeth.
EE had repaired a leak in the suction hose on a co-worker's boom. EE got on the bolter so EE could listen for leaks around the cyclone, troubleshooting. EE told co-worker to drill a test hole so EE could see if there were any leaks. Co-worker swung the boom in pinning EE's foot between the L Arm and the bolter frame. Co-worker was on knees and could not see where EE was.
Employee was pinning 9L and had just finished putting up cable bolts and was walking to the back of the bolter when EE slipped on slack and got left foot under tire of the bolter as it was moving back.
Employee was grinding on a shuttle car, the grinder kicked back, causing a cut on EE's left thumb. Employee started missing 4/29/22 do it an infection.
Employee was standing by the roof bolter operator. The operator said was moving up and the employee and offside operator said ok. When the operator started to tram the Roof Bolter it ran over the employee's right foot. The employee has a contusion on their right foot.
Employee was pinning when a half of head fell out of the mine roof and struck them on the right wrist fracturing the wrist.
Employee was drilling hole for outside pin when the rib rolled off striking them in the lower back. The laceration required stitches.
Employee was installing an anchor pin when EE's hand got caught between 2 pin plates fracturing EE's right thumb. Employee started missing work on 5/6/21.
Employee was truss bolting and had left hand lowering canopy and right hand was pulling up on the mast tilt on the opposite operator side of the truss bolter. The canopy came down hand caught between canopy and metal above levers cutting EE's right hand. The laceration required 2 Stitches.
Employee drilled a hole in the mine roof for a roof bolt. EE lowered the pinner steel and a rock was spinning on the steel. EE reached down to stop the rock from spinning and the rock cut EE's left middle finger. Resulting in three stitches.
Employee had a bottom drill steel hung in the chuck of the roof bolter. Employee was trying to pull the drill sreel out and went to drop boom and hit the rotations causing the drill steel to hit right thumb. Employee's thumb was fractured.
Employee was removing a broke pinner steel that was stuck in the pinner chuck. The employee had a punch wedged between the flinger plate and the canopy post jack and hit the rotation lever, the steel came out of the chuck striking EE in the right jaw.
Employee had drilled the first steel into the mine roof, when the employee let the roof bolter boom down, the boom then came into contact with the second drill steel, which was located in the employee's left hand, causing the steel to penetrate the injured employee's left foot.
Employee caught right index finger between the pinner chuck and the mine roof. This happened while employee was attempting to get a drill steel out of the mine roof. The right index finger was fractured.
Employee was loading roof bolts onto the roof bolter from the scoop bucket. Another employee raised the scoop bucket and the pins began to shift and started falling, the employee kneeled trying to keep the pins from falling out of the scoop bucket. At this time the roof bolter started backing up and ran over the employee's foot.
Employee swug boom over to install cable bolt, had finger on canopy, hit canopy lever instead of boom swing and smashed finger in roof.
Employee was installing roof bolt in place of a damaged roof bolt, using a wrench to try to break off a board on the damaged roof bolt. While lowering boom down the wrench came out of the chuck, striking the employee in the forehead. This required stitches.
Employee was installing a roof bolt in the mine roof, while pushing the roof bolt in the mine roof the pie pan on the roof bolt cut employees left bicep. The laceration required 7 stitches.
Employee had 2 pinner steels stuck together and started beating them apart on the roof bolter and the vibrations from the steels hurt right hand
Employee was installing the second drill steel. The boom of bolter hit the second drill steel causing the drill steel to contact right thumb.
Employee was cutting down gob with the miner in #9 entry, when something came off the head striking EE in the right jaw, this caused a laceration that required 7 stitches.
Employee was removing a two piece drill steel from the mine roof, when the top drill steel fell out of the mine roof striking employees left middle finger against the chuck of the roof bolter. The left middle finger received a small fracture and laceration, the laceration required three stitches.
Employee was moving the roof bolter out of #10 right going to the coal face, and while tramming the roof bolter from the deck. The employee was holding the roof bolter cable to keep it from going under the bolter, EE got their hand between the coal rib and roof bolter deck cutting EE's finger that required stitches.
Employee was operating a dozer while loading a train; a chute stopped up and was not feeding coal. After about 20 minutes of it not feeding the operator's were told to stay off of the feeder. The employee drove his dozer to close as he was feeding the main feeder and the dozer fell in bridge area of the feeder. The employee was not entrapped and he got out on his own.
Employee was installing a 2nd pinner steel, and while holding the steel in his right hand he moved the boom and it pinched his palm between the pinner steels cutting his hand. The cut required stitches.
Employee had just drilled a hole for a roof bolt when he was lowering the boom; he hit a loose steel in tray. The steel swung around and struck employee on the nose, cutting him and the cut required two stitches.
The boom on the truss bolter was extended in the out position. When employee set the ATRS it placed the boom in a bind. Once the employee began to operate the boom it swung in and pinched employee between the ATRS and the boom on the truss bolter.
EE was in the process of cutting a steel cable that was tangled on the D8N dozer at the trash dump. As ee lit the torch the first time it went out. As he was relighting it a buzzing nose and a ignition happened. This caused burns to the left elbow and low arm.
Employee was installing an 8' roof bolt in high top, when lowering the boom of the truss bolter a 2' wrench fell out of the chuck and hit employee in the face cutting him that requiring stitches.
