Dozer slide on ice covered slope after stopping which resulted in the dozer rolling over Damage has not been estimated as of the time.

Employee hit his left elbow on a gear while working on a drill carousel, grinding on a rotation gear. He noticed swelling in his elbow while driving home.

As ee was grading the haul road, he hit a rock with his blade, causing a quick jerk to his back.

As ee was backing up, the dozer "Sit down" hard, causing a quick jerk to the upper middle of his back. EE continued to work until 1:00 PM on 2/10/2012. His diagnosis is pending.

Traming drill up a hill when a Rock undetected in the spoil became dislodged & came into drill, causing allege back injury.

Employee was pushing in a shot with a D11 Cat. Dozer, as he backed up, he ran over a rock, causing a quick jerk to his lower back. He was diagnosed as having a strained lower back muscle.

Bending roof bolt in #2 entry and hurt lower back.

While employee was cutting a tree with a chain saw, a dead limb fell from the tree, striking him in the back. He lost control of the chain saw and it struck him in the left leg, causing 52 stitches. He was transported to the Hazard ARH where he was treated and released.

Employee was pushing a rock with the corner of his blade of his Cat. D10 Dozer when the blade slipped off the rock, causing a quick jerk to employee's back. He was transported to the Hazard ARH where the preliminary diagnosis was a muscle strain.

Operator was digging drainage ditch and was attempting to remove a rock, rock slipped out of excavator bucket and caused machine to whiplash operator from side to side. Operator complained of pain in lower neck and back.

D11 Operator was pushing shot and backed over a rock. The Tractor broke center and sat down backwards with a hard jolt jarring operators back & neck.

Dozer operator wasa pushing shot when dozer slide sideways jerking the operator causing him to strain his neck and upper back muscles.

Operator was backing up when Dozer hit a rock jarring his lower back.

Employee drilled hole in top and steel lodged in hole. As he let down drill pot steel fell, from the hole landing on his left index finger smasheing it against drill chuck.

Spot bolting between spad #1425-1426. In 3 Lt one break inby feeder rib started to roll out causing co-worker to move to avoid rib hitting inch trap causing bolter to move and run over employee's left foot.

Bolt crew was pulling slack when wire holding bolter cable on roof bolt plates began to break resulting cable hitting employee in back of neck.

Operator was pushing shot rock out of shot with D11 dozer. As he was backing up a rock rolled in behind him and he didn't see it. When he backed up on the rock the dozer slide off and jarred him and the machine and hurt his back.

Installing lifting eyes in a D11 converter to set in hole. One of the lifting eyes broke and the other eye was unable to hold the weight and torque fell on left arm causing laceration.

Operator started feeling slight pain after Dozer jarred him while pushing shot on April 21. Towards the end of his shift, he was experiencing moderate-severe pain in lower back. Went to Doctor on April 23 when pain would not ease up.

Dozer was tramming out of shot. Got to the bottom of shot and dropped over a ledge approx 4 feet in height. Operator had his seat belt on. Said it twisted him.

Wash pushing rock, when blade caught, jarring employee. EE immediatley complained of back pain in lower back.

Torch hose blew out burning the left hand middle and ring finger.

777 Truck backed in on field, when truck stopped rock came rolling off side of truck striking dozer on left side.

Employee was driving (777) rock truck being loaded with 992G. Another worker was operating D11 helping 992G & hit 777 truck with D11 blade.

Operator was pushing shot when Dozer backed into a rock causing the dozer to stop suddenly. This sudden jar caused the operator to hurt his lower back.

EE was operating a D11R dozer, pushing rock with the left corner bit of the blade. The section he was pushing broke off of the rock, causing the dozer to jerk forward, at the same time throwing EE forward and then back into the seat.

Operator was cleaning ditch line and berms with excavator bucket, hit tree limb causing it to strike windshield cutting EE's hand.

New 3/4" wire rope cable was being installed on barge winch grabline. While cable was being pulled through an 8" sheave, EE attempted to reposition the sheave. The moving cable caught his right hand & pulled his fingers into the sheave injuring his fingers & hand. One finger was crushed with an open wound. The other fingers were also injured, but not as serious.

Using a torch to cut out master bad bolt when slag popped in and around side shield. Employee did not seek treatment until 07/24/2007 and did not inform employer until 07/31/2007.

Employee was pushing a big rock out of the road, The rock broke and came back hard against the blade. He felt the seat go back causing him to hurt his shoulder. It started burning real bad. He finished his shift. He came back the next shift but was to sore to work. I took him to the ARH hospital in Hazard, KY.

EE was grading road in 2nd gear pulling the ditch back across road. The blade hit a rock causing him to slam forward & hurting his back. He called his foreman on CB radio to come to him. When foreman arrived he asked EE if he needed an ambulance. EE said, "No, I can drive myself to Dr's office." Foreman asked him if he was wearing seatbelt. He said yes.

Pushing forward over rock, dozer spun around with a jerk causing him to hurt his back and leg. Refused medical attention (ambulance), saying he did not want an ambulance. Stated he would drive himself.

Was pushing brush-blade caught, dozer overturned.

Working on piping, nearby heater came on & blew debris in eye.

Operator was cleaning loose rock from highwall when a rock came over his bucket and through the windshield

Tramming miner down hill, track began to pop and skip. He was watching track to make sure it didn't come off when it popped again amd something flew from the track and struck him in the right eye.

Using torch and got slag in eye.

While operating a I/R DM45 highwall drill, the employee was on the road to drill bench and he was thrown into the control levers.

Using a brush grinder and metal fragment went into eye.

Ran over a rock and injured his back.

Using hand grinder, grinding disk cam apart

D8 dozer was pushing shot material out onto coal level. As he was backing up towards the back section, it appeared that the machine turned to its right, backing over the edge of the shot. The result of this caused the dozer to turn over onto a 15' rock wall.

Right hand was drawn into verticle auger while cutting out bad verticle auger bit with acetylene oxygen torch. When the bad bit was cut, pressure was then released causing verticle auger to turn, drawing right hand into auger causing hnad to become lodged with the weight of auger on injured section of wrist and right hand.

18-DEGREE-WEATHER HOSE BLEW ON CUTTER HEAD. EE RAISED LID TO REPLACE THE HOSE. LID FELL ON EE'S LEFT HAND, CUTTING HIS HAND.

EE COMPLAINED WEEK BEFORE WITH HIS BACK. HE HAD BEEN LIFTING AT HOME. WHEN HE GOT ON DOZER HE SAID HE BACKED OVER A SMALL RIDGE OF DIRT & IT JOLTED HIM SOME & THAT'S WHEN SHARP PAIN HIT HIM & HE YELLED FOR HELP.

EE WAS WELDING AND A PIECE OF SLAG GOT UNDER THE HELMET AND BECAME EMBEDDED IN HIS RIGHT EYE.

THIS EE WAS LANSING A PIN OUT & HAD THE ROD LAYING ACROSS HIS LAP. THE ROD BLEW OFF AND THE FIRE BURNED HIS THIGHS AND CHEST.

TWO EMPLOYEES WERE CUTTING TREES FOR A HOLLOW FILL. ONE CUT A TREE THAT FELL THE WRONG WAY. THE TOP OF THE TREE STRUCK THE OTHER, CAUSING NECK, SHOULDER AND LOWER BACK INJURIES.
