Root Cause: It was determined that this was a pre-existing health issue and not a work related issue. Employee had a artery blockage. Employee did not pass away on project site. Employee passed away at the hospital. MSHA was notified immediately on November 6, 2014.

Employee had been cutting wood with Skil saw earlier and some sawdust was on clothing. A gust of wind blew debris in employee's left eye. The employee was wearing safety glasses at time of occurrence.
EMPLOYEE WORKING TO PALLETIZE FILTER CLOTHS WHEN HE NOTICED HE HAD BEEN BITTEN BY A TICK.
This is not an accident. Emp was working with supervisor welding, wearing all PPE, when supervisor noticed emp's coloring not right. Supervisor told emp to take off PPE and sit down. Supervisor noticed emp's condition did not improve & proceeded to take emp to local EMR & he was diagnosed as having a heart attack.
Driver was picking up a container of clay to take to port. After he hooked truck to container and was pulling away he noticed container had come loose and fell on landing gear. Other j. driver was assiting him by backing his truck under side of container to lift it up to enable driver to rehook container. The second driver noticed that EE had fell to ground. cpr but no avilable.
Employee was walking to loader to fill feeder hoppers when he felt stinging sensation on upper left arm. He lifted shirt sleeve and saw brown spider. He killed spider. The next day he still had stinging sensation and a small lump where spider had been.
