Employee said that he exited the #1 and #2 Kilns control room to the # 1 kilns firing deck. He felt a twitch or pinching feeling to his left knee. He returned to the control room. When he would put pressure to the heel of his left leg, he felt sharp pains to his left knee.

EE was watching his Rotary Kiln the wind blew fine gritty material underneath his safety glasses scratching his left eye.

The injured miner was replacing a trust roller on the #1 kiln upper pier. The wind was blowing foreign materials around, when the miner wiped his brow, a piece of foreign material got into his eye.

The injured miner was preparing to change a trust roller on the #1 kiln upper pier. The miner was clearing material away, and the wind blew foreign material into his eye.

Employee clocked in at 7:00 a.m., he had been feeling sick since 3:30 a.m. that morning. He came to work anyway to learn he wasn't feeling any better. Another employee helped him to the office where he described to us that he was having chest pains and difficulty breathing. We called 911 for assistance.

On Monday 3/9/09 maintenance crew was doing repair work in one of our kilns. At this time no accident had occurred. On Tues morning the employee woke up with a crick to the right side of his neck. He alleged that the crick in his neck came from work being performed at the Carolina Stalite plant. He didn't report it until 3/16/09. Sent him to the dr. the same day.

The EE exited the #1 and #2 control room to wash down the #1 kiln firing deck, when the EE knelt down to wash between the kilns firing hood and the control room , he said he heard amd felt his knee pop.

EE was lubricating rotary kiln equipment. Around 6:30 he had dinner, then around 8:30 ee & co-worker met with their supervisor in the control room. He said he felt hot, so he took off one of the shirts he had on & stood outside of the control room to cool off. His supervisor went with him to the breakroom to relax for a while. He started having chest pain, another ee called 911.

EE WAS CLEANING OUT SOME DRY LIME CLOTS OUT OF THE #7 KILN DROP BOX. THE WIND WAS BLOWING TOWARD HIM AS HE WAS CLEANING OUT THE BOX. THE WIND BLEW SOME LIME INTO HIS RIGHT EYE.

EMPLOYEE WAS WALKING ACROSS OPEN AREA, WIND GUSTED BLOWING DIRT PARTICLE IN HIS EYE. EMPLOYEE SUFFEREN SCRATCH TO CORNEA OF RIGHT EYE.
