Employee experienced chest pain and was taken to the ER were it was determined EE had a heart attack.
Employee was outside on a very windy day and got debris in right eye which they could not get rinsed out with saline. This caused a scratch across the top of pupil.
Walking down stairs and neck started hurting. Went to doctor on 1/26/16 and was told to ice neck. EE decided to take the rest of the day off using a vacation day so ee could ice neck.
Worker was changing bearing when guard that was leaning on handrail was blown by wind onto worker's back. He reported the incident & was fine (little pain) until 5/5/10 when great pain was reported. He was sent for medical evaluation. Doctor wanted 2-weeks light duty for worker. Worker has reported history of back issues/pain from recreational activities outside of work.
Working on a dryer, he stepped outside to get additional tools and was stung on top of right ear by bee.
THE EE DID NOT HAVE AN ACCIDENT. HE WAS COMPLAINING OF PAIN IN HIS BACK (OVER HIS SHOULDER) & HIS CHEST. THE EE DID NOT WANT TO GO TO THE HOSPITAL, NOR DID HE WANT AN AMBULANCE CALLED. ANOTH ER EE TOOK HIM TO THE HOSPITAL ER. SOMEWHERE DURING THIS TIME, THE EE HAD A HEART ATTACK. THIS IS BEING REPORTED UNDER MSHA GUIDELINES. HE IS CURRENTLY OFF WORK.

IT WAS WINDY OUTSIDE THIS DAY. THE INJURED INDIVIDUAL WAS SHOVELING SAND. THE WIND WAS BLOWING SAN DNAD PARTICLES INT EH AIR AND SOMETHING WENT INTO HIS EYE. THE INDIVIDUAL THOUGHT IT WS SAN D IN HIS RIGHT EYE. HE WENT TO AN EYE CLINIC & A SMALL PIECE OF STEEL WAS REMOVED USING SOME TYPE OF MEDICAL INSTRUMENT. NO MEIDCATION.
THE WIND BLEW SAND AND RUST OFF A BEAM ABOVE THE WINDOW. THE INDIVIDUAL WAS CLOSING WINDOWS IN THE BAGGING DEPARTMENT TOWER.
INDIVIDUAL WAS WELDING IN AN AREA WHERE BLOWING SAND, DUE TO WIND, IS FREQUENT HAZARD AND OBJECT BLEW INTO EYE. INDIVIDUAL WAS GIVEN FIRST AID AND PRESCRIPTION.
