The EE was standing next to the cart located at the bottom of the preheater tower, having a conversation with the Manager. EE was wearing eye protection, but a gust of wind blew calciner dust off the deck above the EE, and into EE's right eye. EE reported it as a first aid, but the next morning due to irritation, went to the Dr. EE was prescribed anti-inflammatory eye drops.

EE was stepping down the metal stairs from bucket wheel on reclaimer. EE went to turn to walk the to the ladder, the EE's left knee gave out and caused EE sharp pain in EE's knee. On 3/20/23 EE said knee was still sore, and a little swollen. EE went to Doctor, and returned to duty with restrictions. No kneeling, squatting, climbing.

EE was an over the road truck driver for a trucking company, not a miner. EE passed away from as yet undetermined personal medical factors while in the truck cab on the scale at the Camp Verde Gyp Mine. The death was not related to any mining activity, event, incident or action. EE's last CDL medical clearance to drive was on 01/31/2022 and did not expire until 01/31/2023.

EE felt a pop and pain in right ankle when standing up from desk. Suspects related to a F/A only on the same ankle on 04/11/22. Requested medical evaluation. EE was diagnosed with a strained calf muscle and ankle ligament and returned to work with a brace and restrictions. Treatment is on going.

EE was doing a PM on DBC-404 when the wind blew something behind EE's safety glasses and into EE's eye. EE was unable to flush it out because EE rubbed the eye before using eyewash. EE was sent to the eye Dr. who removed a foreign body with forceps, prescribed antibiotic eye drops and released EE to work.

EE was discovered unresponsive laying on the floor in the restroom. Site personnel began CPR and called EMS. EMS took over life saving efforts upon arrival but were unable to revive EE.

EE walked a few feet to pick up a caulking gun laying on top of a screw housing (EE did not have to bend over) and when EE turned to walk back the other direction EE felt a pain in the middle back. EE was sent for medical evaluation, diagnosed with a back strain and released to work with restrictions.

EE reported noticing stiffness in left knee during shift (no accident/ event) on 22JAN17 as a first aid only and wanted to wait and see if it went away. EE was still experiencing intermittent stiffness and discomfort and was sent for medical evaluation on 07FEB17. EE was diagnosed with a strain/sprain of the left knee and released to work with restrictions.

EE Was bent over showing mech. a problem with his Haul Truck when he felt a sharp pain in lower back. EE was sent for medical evaluation, diagnosed with lumbar strain. EE was released to work with restrictions.

While sitting he turned his head to acknowledge a fellow crewman and felt tightness in his neck and shoulders.

While replacing a cotter pin he tweaked his back when he twisted to get into position.

Stepped put of cart and his back went out.

Spider bite. While driving the 980G loader, pushing up the gyp shed, he noticed a burning pain and swelling on his left leg.

He was moving some rock and got stung by a scorpion on his left hand middle finger.

AS HE WAS WORKING AROUND BM304 IT WAS WINDY AND DUST GOT INTO HIS EYES.

Ee was filling out a truck log at the security office when a bug flew into her eye. Ee went to doctor & was given a prescription.

EMPLOYEE COMPLAINED HE WASN'T FEELING WELL SO HE TOLD HIS SUPERVISOR THAT HE CALLED HIS WIFE TO PICK HIM UP. SHE DID SO & LATER TOOK HIM TO THE E.R. HE HAD TO HAVE HEART BY-PASS SURGERY.

WHILE CHANGING OUT BAGS IN A DUST COLLECTOR, CEMENT DUST FELL INTO HIS RIGHT EAR.

WHILE HANGING NEW BAGS IN DUST COLLECTOR, DUST FELL OFF ABOVE RAFTER INTO RIGHT EYE. HE WAS WEARING GOGGLES AT THE TIME BUT SOMEHOW DUST GOT BEHIND THE GLASS.

NOT KNOWN THE EXACT CAUSE OF ACCIDENT. EE WAS WORKING ON CLINKER STACKER TRAILER AT BELT CONVEYOR 211. HE HAD TO GO UP AND DOWN STAIRS, AND TRAILER HAS A SLOPED RAMP. AFTER BREAK THE PAIN SEEM ED WORSE. EE HAS A HISTORY OF BACK PROBLEMS.

WHLE GATHERING THE BUFFALO TO PUT IN SMALLER ENS ACCIDENTLY DISTURBED A WASP NEST

WHILE WORKING IN THE MACHINE SHOP, A GUST OF WIND WENT THRU THE BUILDING AND BLEW DUST AND A SLIVER OF METAL IN RIGHT EYE.

WHILE EMPTYING COAL REJECT DISCHARGE WHEELBARROW #2 COAL MILL A BUG FLEW UNDER SAFETY GLASSES AND BOUNCED OFF LENS INTO THE LEFT EYE. THE BUG WAS LODGED INTO THE CORNER OF THE EYE. AFTER DOCTO R REMOVED THE BUG THE EYE WAS FINE. NO PERMANENTDAMAGE.

WHILE PERFORMING CLEANUP DITIES EMPLOYEE WAS STUNG ON RIGHT ARM BY A TIGER BEETLE.

WINDY, DUSTY CONDITIONS. EMPLOYEE GOT DUST IN HIS EYES.

WORKING IN WINDY GUSTY CONDITIONS, EMPLOYEE RECEIVED FOREIGN BOD IN R/EYE.

WHEN EXITING THE CAB OF A DART LOADER, A WIND GUST BLEW THE DOOR SHUT ON EMPLOYEE'S HAND.

WHILE WORKING IN BULK LOADING AEAD WIND GUSTED BLOWING FOREIGN OBJECT INTO EE EYE.

HEART ATTACK NON WORK RELATED

EMPLOYEE WAS TARPING CLINKER PYLE. HIS FEET BECAME TANGELED UP IN TARP WIND CAME UP, EMPLOYEE FELL 6-8' LANDING ON BACK, EMPLOYEE SUFFERED BACK STRAIN;, WAS PLACED ON LIGHT DUTY & INSTRUCTED T

EMPLOYEE REPORTED TO ME ON 3-3-93, THAT HE HAD BEEN EXPERIENCING IRRITATION IN HIS RIGHT EYE LIKE SOMETHING WAS IN IT. HE STATED THAT HE COULD NOT PIN POINT A TIME WHEN HE BEGAN EXPERIENCING T HE IRRITATION. HE ALSO STATED THAT HE DID NOT KNOW IF HE HAD GOTTEN SOMETHING IN HIS EYE AT WORK OR AT HOME.

WIND BLEW SLAG INTO RIGHT EYE.
