EE is believed to have received insect bites on 10/11/25. On 10/14/25, the EE sought treatment due to swelling, redness and itchiness at the bit site. EE received a steroid injection, prescription antibiotics and was advised to go home and rest. EE went to urgent care for follow-up on 10/16/25. Operator notified on 11/17/25, when urgent care invoice was received.

Mostrando los 3Mine EE annual hearing test came back over the acceptable level of 25db on 8/9/17. EE was re-tested on 8/25/17 per the regulations and again was over the 25db level. EE continues EE's enrollment in the Hearing Conservation Program (HCP) and all engineering & administrative controls are being reassessed.
Employee (EE) visited clinic on this 8/13 with complaint of stiff/sore neck resulting from work performed on 8/10. On 8/10 EE was working on Primary Crusher removing, cleaning, and replacing wear plates. EE was working in an awkward, stooped position. On 8/13 Dr. diagnosed neck sprain/back strain and ordered physical therapy. EE returned to work same day.
