Clearing of 2D silo discharge gate watching for the material to free up and had safety glasses on at the time.

Foreign body was imbedded in employee's right eye when material from overhead blew into his eye. He was wearing safety eyeglasses. FB removed by doctor. EE returned to work with no lost or restricted workdays.

Employee was unloading a delivery of pallets from a van. Wind blew trash into employees left eye, causing redness and irritation.

Non-work related ailment, employee stopped taking blood pressure medication causing his blood pressure to be unstable.

An employee claims he was bitten by a spider on 10-10-2006. The affected area developed a staph infection after several days. The employee was admitted into the hospital on October 14th, 2006.

EE had been working in the parts warehouse. He felt a tightening sensation & pain in his chest as he sat down to drink a Gatorade. The supervisor was notified. He was taken to the local ER, where he was treated as a heart attack victim. Following tests, 3 stints were inserted into the coronary arteries. He is much improved & will be returning to work soon.

EMPLOYEE SAID HIS BACK FELT TIGHT SO HE BENT OVER TO STRECH AND HE SAID IT FELT LIKE SOMETHING GAVE IN HIS BACK.

EE WAS PERFORMING NORMAL JOB DUTIES. NO SPECIFIC INCIDENT. EE CLAIMS THAT THIS IS A REOCCURRENCE OF BACK INJURY OF 2/27/96. AFTER AN INVESTIGATION BY OUR WORKERS' COMP CARRIER, KEMPER, THEY AG REE WITH EE THAT THIS IS A REOCCURRENCE OF HIS FORMER BACK INJURY. THIS DECISION WAS REPORTED TO US ON 4/15/97.

EMPLOYEE INDICATED THAT A GUST OF WIND BLEW SOMETHING INTO HIS EYE.
