Employee was squatting, working on a rail guard on the crusher when they felt a pop in knee. After the initial doctors office visit, EE was prescribed self care. The knee did not improve and doctor requested an MRI. The MRI revealed a torn meniscus that would require surgery to repair. Employee underwent surgery on 10/7 was returned to modified duty following surgery on 10/10.

Employee has had back issues previously, started hurting again & having leg pain while working. He requested to go to the dr on 7/23/14. He continued to work his full work duties while continuing to see dr and going through physical therapy. The dr decided to do surgery 10/7/14 and no work at least until 11/10/14. When employee can rtd this form will be updated
