EE was removing an extension cord from the cyclone. Dust blew into right eye around safety glasses. EE washed the eye out with eye wash solution but said that the eye was still feeling like something was in it.

EE was removing blower, when he squatted down & felt right knee pop. EE continued working for following days until dr. prescribed no climbing restriction.
EE reported (3 days later) back discomfort, and was unsure of activity that caused discomfort. Discomfort did not restrict EE job assignments until 2/4/13.
National Cement was notified by rescue services, that an individual jumped off of highwall of non-active flooded quarry. The individual did not resurface and recovery actions were initiated. Immediately called MSHA hotline, and explained that the individual was trespassing on property, and was not a work related fatality. MSHA inspector arrived and conducted investigation.
EE felt pain in abdomen while at work, but when reported, was not sure of activity that caused discomfort. Physician diagnosed as hernia, and EE underwent surgery on 8/16/12.
EE took medical leave to have disc surgery. While awaiting surgery, employee filed for work comp. and received doctor statement of work related cause of medical condition. There has been no specific accident or date related to this condition.

At the end of the day the EE went to the shipping office to put on his jacket. When he put it on he felt a sting from a spider.
