Employee was operating a trackhoe moving a log and bees came out and stung him ove 30 times on his head and arms. He proceeded to the emergency room. Tests were performed but since he is not allergic, he was released to take Benadryl and return to work.

Wind blew door shut of lube truck on right 4th digit, latching finger in door. Finger lacerations and slight fracture of digit.

Employee was cleaning up around the plant with a Skidsteer and was bitten by a spider. He was treated with antibiotics. The infection got worse and he was ordered to stay home and keep still.

EMPLOYEE ALLEGES HE WAS HAULING LIME AND SOME DUST BLEW INTO HIS EYES. DIDN'T REPORT UNTIL 7/29/02. EYE DOCTOR COULDN'T FIND ANYTHING IN HIS EYES BUT APPLIED DROPS TO RELIEVE IRRITATION.

EMPLOYEE HAS LUMBAR STRAIN. SHE IS A QUALITY CONTROL INSPECTOR AND SHE DOESN'T KNOW EXACTLY WHAT SHE DID. SHE ESTIMATES HEAVY LIFTING OR REPETITIOUS WORK OR GRABBING THE DOOR TO KEEP IT SHUT W ITH THE WIND BLOWING. SHE SOUGHT HER OWN MEDICAL CARE AND DID NOT REPORT IT TO US UNTIL 5/13/02.

THE EMPLOYEE IS MEXICAN AND COMMUNICATION IS ALMOST IMPOSSIBLE WITHOUT AN INTERPRETOR. HE ALLEGES HIS EYE WAS SWELLING BECAUSE OF THE HEAT. HE COULD NOT GIVE A DATE FOR INJURY. HE WENT TO THE DOCTOR WITHOUT TELLING HIS SUPERVISOR AND A FEW DAYS LATER REPORTED IT AS WORK RELATED. HE SAW THE DOCTOR ON 7/13/00 AND BROUGHT BACK A NOTE STATING NOT TO RETURN TO WORK UNTIL 7/16/00.
