On 10/16/24 EE was supporting verimill project. EE was walking up & down stairs for 1 to 2 hours. Began to feel some discomfort. On 10/22 EE was issued restrictions, had first appointment on 10/31/24.

Mostrando los 28An employee was eating almonds while on a break and they cracked a tooth. They went to the dentist to have the tooth repaired.
Employee had just finished lunch, grabbed a water bottle on EE's way back to EE's position on the job. EE collapsed but was awake and confused.
Employee stood up from kneeling position and felt strain to right knee. Medical diagnosis MRI determined tear to the meniscus.
EE was inspecting the Tailings Booster station containment area. EE bent over to access a sump pump and EE began to feel pain in EE's back and under EE's right shoulder blade. EE was seen at clinic and was issued restrictions.
EE was found unresponsive behind the wheel of a parked loader. Loader was in a secured position. EE was taken off loader and resuscitation measures were performed. EE was taken to the clinic to an ambulance where they were taken to another medical facility where EE was pronounced deceased.
EE was working on #9 Tertiary Dust collector when ee felt something crawling inside shirt sleeve. It bit ee. EE squashed the bug & it rolled out of sleeve. EE went home w/o reporting. The next morning ee woke up to find that arm was swollen & the bite site had become infected. EE had pain in arm. EE sent to clinic, taken off work & given antibiotics for infection.
EE was descending a stairway in the B-regrind when right knee gave out. EE was able to put hands out to grab the handrails and to lower self on the stairs. EE sustained right knee sprain.
FMI personnel were notified by a citizen that there was a deceased person near the water tanks. Appears to be an illegal individual. Pima County Sheriffs Dept. and Pima County Coroner office processed the area and departed with the body. Individual was wearing camouflage clothing, a baseball cap and wearing carpet shoes. Appears person has been deceased for quite some time.
Employee notified Security personnel that a (non-employee) was appeared to be unconscious sitting in a vehicle in the employee parking lot. FMI EMT's investigated and found the person slumped over in car. Green Valley FD was called and the subject was taken to the hospital where that person was pronounced deceased. The subject was a Silver Alert.
Employee was working in Mine Engineering office when ee felt a bite by an insect on left shoulder. EE visually inspected the area noticed redness and swelling. The next day the "itchiness" continued. EE noticed large red spot where the bite was located. Employee taken to clinic; bite area was infected. EE was treated for infection - employee nursing.
EE was getting into haul truck when she smelt a strong perfume odor. The odor caused her to sneeze and cough causing chest pains.
Employees were working in A2/A3 dropbox area when bees were reported in area. EE was instructed to wait in the vehicle while bee removal was contacted. He was stung during this time. EE developed a reaction to the bee sting area. EE sought medical treatment by physician to combat the reaction.
EE was working at his desk in the training file room. He was sitting, organizing some paperwork. He leaned over to pick up something that had fallen on the floor and attempted to sit back up when he felt a sharp pain in his lower back.
EE was sitting in a computer chair on 4/18/2013 and when a vehicle pulled up to the gate he pushed himself back from the desk with his right leg. As the chair rolled back a wheel stuck to the plastic floor mat causing the chair to come to a sudden stop. EE felt pain. After seeing a ortho specialist he went out lost time 04/26/2013
EE was leading stretching exercises for both the Utility and Labor crews prior to the beginning of the work day. During the stretches he felt a "pop" in his right knee and fell to the ground.
Employee was working on shovel motivator at 47 Shovel on 03/11/2011 she was stung by a bee on right forearm. On 03/18/2011 the employee informed her supervisor that the area where she was stung was red and growing. She was seen by a physician on 03/18/2011 at which time it went from First Aid to medical treatment. Physician administered antibiotics through injection and orally.
Employee stated he did not know how it happened but he did notice that it hurt on his way back to the Maintenance Shop from the job area. Eye began to hurt. Employee flushed eye. Employee went home, and went to sleep; when he awoke his eye was still hurting. Employee went to medical facility to have eye checked out; doctor found piece of metal in eye and it was removed.
Employee states he injured his back by standing up to walk. Employee was taken to clinic and put on restricted work duty.
While grinding and air arcing on new truck bed, welder felt debris blow into his eyes (weather conditions very windy). Left eye became irritated; condition of eye worsened. EE treated by doctor on 11/16/09.
EE assisting in mill liner job. Liner on dolly nudged EE on the leg, startled him, he quickly turned, causing pain in his lower back. EE did not require medical attention at that time. He requested to see dr. and was treated on 9/11/09. EE continued to feel discomfort; on 9/18 re-examined and scripted PT.
Supervisor noticed employee was disoriented, he then noticed that her hard hat was lying on the ground. She was transported into the hospital.
Employee claims he noticed his right leg itching and numb, and rash on right thigh and swollen. He claims it may have happened when he was working on a motor and noticed small, yellow spiders in the area.
While on break from a training session, employee was standing outside the training room when a gust of wind blew particles into his eye. This resulted in an irritation to his left eye.
Employee knelt down by the elevators at the mill moly plant and strained his right knee when he stood back up.
Employee was unloading a rail car of grinding balls when he complained of his left shoulder being stung or bitten by an unknown insect. An hour later a rash developed.
Employee knelt down and felt pain in his left knee.
EE CLAIMS HE WAS WALKING FAST ON FLAT CEMENT SURFACE AT THE FINE ORE TRIPPER DECK WHEN HE FELT HIS KNEE POP & HE WAS UNABLE TO CONTINUE WALING
