Employee was in the kiln shack grabbing a pair of gloves & after a while of wearing them their hand began itching and had swollen up. Harvey believes the glove had a insect in it that had bit them irritating their hand.

EE called another EE around 7:00 am as EE was pulling into the plant explaining IE was not feeling well and a bit lightheaded, When EE went up to check on IE who seemed out of it and confused at which point immediately had other EE call 911 between 7:15 & 7:30 am 01-17-25.

The doctors classified it as a mini-stroke.

Employee washing out in the kiln tunnel under the cooler while doing so they crouched down to move under the cooler & when they stood up, they said they felt their back catch & pop sending pain through left leg immediately. Employee stated while standing the pain in their leg was mild & when sitting it would become worse.

Injured employee was helping change out a roller on the #1 kiln. The wind was blowing hard yesterday. EE felt something in right eye, like it was scratched. That afternoon eye became sore, ee seen a speck of something on eye. EE tried to remove it but was unable to. EE reported this to supervisor the following morning.

Employee begin to have chest pains before employee starting working.

EE was weighing in and out customer trucks. EE began to have chest pains, then later numbness to EE face and hands. More so to the right side.

Employee said that he exited the #1 and #2 Kilns control room to the # 1 kilns firing deck. He felt a twitch or pinching feeling to his left knee. He returned to the control room. When he would put pressure to the heel of his left leg, he felt sharp pains to his left knee.

EE was watching his Rotary Kiln the wind blew fine gritty material underneath his safety glasses scratching his left eye.

The injured miner was replacing a trust roller on the #1 kiln upper pier. The wind was blowing foreign materials around, when the miner wiped his brow, a piece of foreign material got into his eye.

The injured miner was preparing to change a trust roller on the #1 kiln upper pier. The miner was clearing material away, and the wind blew foreign material into his eye.

Employee clocked in at 7:00 a.m., he had been feeling sick since 3:30 a.m. that morning. He came to work anyway to learn he wasn't feeling any better. Another employee helped him to the office where he described to us that he was having chest pains and difficulty breathing. We called 911 for assistance.

On Monday 3/9/09 maintenance crew was doing repair work in one of our kilns. At this time no accident had occurred. On Tues morning the employee woke up with a crick to the right side of his neck. He alleged that the crick in his neck came from work being performed at the Carolina Stalite plant. He didn't report it until 3/16/09. Sent him to the dr. the same day.

The EE exited the #1 and #2 control room to wash down the #1 kiln firing deck, when the EE knelt down to wash between the kilns firing hood and the control room , he said he heard amd felt his knee pop.

EE was lubricating rotary kiln equipment. Around 6:30 he had dinner, then around 8:30 ee & co-worker met with their supervisor in the control room. He said he felt hot, so he took off one of the shirts he had on & stood outside of the control room to cool off. His supervisor went with him to the breakroom to relax for a while. He started having chest pain, another ee called 911.

EE WAS CLEANING OUT SOME DRY LIME CLOTS OUT OF THE #7 KILN DROP BOX. THE WIND WAS BLOWING TOWARD HIM AS HE WAS CLEANING OUT THE BOX. THE WIND BLEW SOME LIME INTO HIS RIGHT EYE.

EMPLOYEE WAS WALKING ACROSS OPEN AREA, WIND GUSTED BLOWING DIRT PARTICLE IN HIS EYE. EMPLOYEE SUFFEREN SCRATCH TO CORNEA OF RIGHT EYE.

EE HAD WALKED OUT THE DOOR OF THE #1 KILN BURNERPLATFORM TO LOOK AT THE KILN WHEN THE WIND BLEW SOMETHING IN HIS RT EYE [WEARING SAFETY GLASSES]

EMPLOYEE WAS USING A WATER HOSE TO WASH OFF THE COOLER DECK WHEN THE WIND BLEW SOME FINE GRIT INTO HIS LEFT EYE.

EMPLOYEE WAS WALKING ACROSS PLATFORM & THE WIND BLEW A FLAKE OF METAL INTO HIS LEFT EYE. EMPLOYEE REMOVED OBJECT & RUST RING FORMED WENT TO DR. ON 3/23/94.

EMPLOYEE WAS WASHING DOWN MATERIALS AT 5 & 6 DISCHARGE BELTS WHEN THE WIND BLEW SOMETHING IN HI SLEFT EYE SCRATCHING IT. EMPLOYEE STATED THAT HE DID HAVE HIS SAFETY GLASSES ON.

EMPLOYEE HAD FINISHED SHOOTING IN #2 KILN WHEN THE WIND BLEW FOREIGN BODY IN RIGHT EYE.

THIS CASE HAS BEEN REOPENED FROM AN ALLEDGED BACK INJURY ON 8/28/88 ANDD HAS REACTIVATED A RESTRICTED WORK ACTIVITY

COMING BACK UP CATWALK FORM COAL MILL BACK GAVE AWAY.

KILN FOREMAN WAS WALKING ACROSS #1@2 KILN CONTROL BURNER PLATOFRM WHEN THE WIND BLEW A PICE OF FLY ASH IN LEFT EYE.IT WAS REMOVED WITH A MEEDLE KNIFE.

EMPLOYEE WAS WALKING ACROSS THE # 3 BURNER PLATFORM WHEN A FLAKE OF BURNT ROCK BLEW INTO HIS LEFT EYE EMPLOYEE WAS WEARING HIS SAFETY GLASSES
