Employee continued to work without restriction until seeking medical attention on 4/24/2026, and began missing days after that. Injury was not the result of a training or compliance issue.

Moving passenger vehicle. The closing door struck the left side of EE's head, knocking head and most of body into the car. The EE received a minor cut on the R side of head, eyebrow area, but L leg, below the knee, was still outside the vehicle. The vehicle continued forward, still in contact with 12" barrier post, door closed, causing both bones in EE's lower leg to be fractured.

Their right hand was caught between the locomotive body and rib. The result of the injury is amputation of the middle and ring fingers to the middle phalanges and amputation of the little finger to the distal phalange.

The EE stated: while driving a 4 man diesel EE ran upon a small gob pile, while backing up off the pile, the steering wheel spun around striking EE's left wrist
Employee was loading a loader onto a dolly and the loader slid off the dolly and cribbing. The impact from the loader bottoming out resulted in the slotted canopy to shift and pinched the employee against the controls of the loader.
