Employee was welding when the welder stopped working, employee raised hood and received flash burns when welder suddenly arc. Mining systems was not a factor, compliance with rules and regulations was not a factor, PPE was a factor.

Employee was using jack hammer when the jack hammer tip slipped off rock and landed on top of employee's right foot. PPE was not a factor, mining systems was not a factor, compliance with rules and regulations was not a factor.

Employee was carrying 20' beams through mud. The employee got EE's foot caught on a cable bolt that was sticking out of the mud, causing EE to fall with the beam in EE's hands. Employee fell to the ground, crushing EE's left hand under the beam and EE's body weight.

IE was a passenger in a mantrip that was travelling into a track spur. The mantrip struck a dolly that was unseen by the driver, and IE's back struck EE's seat hard. Employee continued to work without restriction until seeking medical attention on 4/24/2026, and began missing days after that. Injury was not the result of a training or compliance issue.

Employee was loading waterline onto scoop for belt move. When one of the water lines pipes slid off the scoop and struck EE's foot. PPE was not a factor, mining systems was not a factor, compliance with rules and regulations was not a factor.

An unplanned roof fall occurred on 3/5/2026 at 11:50 am. The fall measured 16' long by 13' wide by 8' high. The fall occurred in #1 entry just in by 90 crosscut in the 6 South Mains. The area was dangered off and will be cleaned up.

Employee had finished bolting on EE's side and went to assist EE's bolting partner who was struggling with setting EE's drill steels out of the top. EE asked EE's bolting partner to lower the drill pod a little bit but instead EE brought it down all the way on top of the employees right foot.

Employee and coworkers were jacking up the belt to change rollers, when the chain attached from the come along to the belt structure snapped causing the structure to fall and the employee to fall to the ground with it.

A Trip of cars was being moved when it struck a motor and dolly at the end of the track, causing the dolly to come off track and hit fan. The fan moved sideways, causing the fan to pinch miner between coal rib and fan.
