EE was backing down the grizzly ramp. The loader proceeded to powerslide and came to a halt at the bottom of the ramp. At that time, EE felt a "pop" in EE's neck. At 9:00am EE reported feelings of "nausea and motion sickness" to the shift foreman. On 9/25/2017 EE was diagnosed with a neck injury.

Mostrando los 8EE was emptying bin into a bulk sack inside plug mud packaging area. Forklift operator approached bulk sack holding frame, placed forks in pallet, and tilted them forward. Left edge of forklift backstop struck EE fingertip, which was resting on metal holding frame.
EE did not report until 4/16/2013 that he had jarred his back sometime in December 2012 while operating a front end loader. On 4/16/2013 he told his supervisor about the incident and wanted to go to the doctor. EE was placed on restricted duty awaiting the results of an MRI. Upon receiving results all restrictions were removed and EE returned to work at full capacity.
EE reached around a guard to clear material conveyor without following proper lock out tag out procedures. Hand was caught between the tail pulley and idler conveyor. He was able to pull a stop cord stopping the conveyor, but remained with his hand caught. He had to wait assistance to be removed from the conveyor. His hand received abrasions as a result of this incident.
Employee reached around the conveyor guard and grabbed a plug mud bag between the conveyor belt pinch point and the bag roller. Upon doing this his hand was pulled into the pinch point between the conveyor belt and the bag roller.
Employee was reaching to pull a bag off a conveyor when his glove got caught between the conveyor and the deck, pinching his left hand and wrist. Employee suffered a bruised left hand and a wrist abrasion.
Employee exceeded safe speed limitation for fork lift while making a sharp turn causing the fork lift to turn on its side. Weather and slippery conditions were not a factor. Employee was wearing his seat belt.
EE HAD ACCESSED THE TOP OF THE BAG PRESS WITH A STEP LADDER TO MAKE ADJUSTMENT. THE PRESS WAS NOT DE-ENERGIZED AND WHEN THE EE WAS DESCENDING THE LADDER HIS HAND CAME IN CONTACT WITH THE BELT ON THE PRESS, RESULTING IN HIS LEFT ARM BEING DRAWN INTO THE PULLEY, TRAPPING AND BREAKING THE EE'S FOREARM.
