The moving Ramcar caught the roof bolter cable hanging from bullhorns, pulling the cable down. The moving cable forcibly struck the employee standing on the Ramcar, knocking that employee down on the Ramcar.
While riding in the back of a mantrip on a road that had been excessively watered, the employee struck against the mantrip shell when EE's vehicle slid into another mantrip that was stopped in the entry, resulting in back pain and a sprained ankle.

EE STATES HE WAS ROCK DUSTING THE BELT ENTRY USING SV-5 WHEN THE VEHICLE GOT STUCK.TO BACK THE VEHICLE UP,HE TURNED AROUND IN OPERATOR'S SEAT & PLACED HIS RIGHT HAND ON TOP OF THE FUEL TANK BEHIND HIM.WHEN VEHICLE MOVED,IT SLID TOWARDS BELT STRUCTURE,CATCHING HIS RIGHT INDEX FINGER ON A RETURN BELT ROLLER HANGER, RESULTING IN A LACERATION THAT REQUIRED SUTURES.
EMPLOYEE STATES HE WAS IN FORK TRUCK BASKET INSTALLING CRIB. AS BASKET WAS LOWERED, EMPLOYEE HIT RAIL ON BASKET, STRIKING HIS LEFT WRIST ON RAIL, CAUSING A LEFT WRIST FRACTURE.
Was operating a fork truck, as he went around a corner the rear tire caught a drill steel that was leaning aginst the rib pulling it forward striking him in the hard hat. The hard hat was forced down and caused a laceration to the L-side of his head.
EMPLOYEE STATES HE WAS DRIVING RAMCAR, PUSHING ANOTHER RAMCAR UP THE HILL. HIS MACHINE CAUGHT ON THE TAILGATE OF THE OTHER, RISING HIGHER AS THEY DROVE. WHEN IT FELL OFF, IT CAUSED EMPLOYEE TO BE BOUNCED AROUND CAB, RESULTING IN FRACTURED VERTEBRA.
EMPLOYEE WAS INADVERTENTLY STRUCK BY THE BUCKET OF THE SCOOP.

EMPLOYEE STATES HE WAS HAULING SUPPLIES WITH FORKTRUCK. HE STRUCK A BUMP IN ROAD CAUSING HIM TO STRIKE HIS SHOULDER AND ARM AGAINST CAB FRAME. SINCE THE DATE OF INJURY, NO RECORDABLE MEDICAL TREATMENT HAS BEEN PROVIDED; HOWEVER, EMPLOYEE INCURRED LOST TIME DUE TO THE INJURY ON 03/26/2008.
Employee was a passenger on SV-11. The vehicle engine stopped. Before the brakes set, the vehicle hit the rib causing the engine cover to strike the employee's legs, breaking both legs.
Employee states he was pushing a Ramcar up a hill with the Ramcar he was operating. He pushed it up the hill his car climbed up on the car being pushed. When the car being pushed caught traction it pulled out from under the car doing the pushing and it dropped to the floor. The employees hard hat come off and he struck his head against the canopy resulting in a laceration.
Employee was delivering samples to lab on 4-wheeler. Weather was a factor. Was coming into a corner when machine started to slide sideways catching and causing machine to roll over.
Employee states he lifted the forks, lost control with one hand allowing forks to fall catching his finger between forks and frame.

The ee came around a corner and there was a 5 yard loader pulling out onto the roadway, as the ee braked and tried to change lanes the loader did the same. The truck hit the load damaging the right front side of the vehicle. the driver hit the mirror with his head causing a lacertion
EE STATES THAT HE WAS CROSSING THE PARKING LOT TO COME TO WORK AND ANOTHER EE ARRIVING FOR WORK DID NOT SEE HIM & STRUCK HIM WITH VEHICLE. THIS DID NOT TURN INTO A LOST TIME ACCIDENT UNTIL 10/06/04
Employee states he blocked in the mantrip throttle to build pressure on compressor. When he drove forward with blocked throttle, the mantrip lunged forward striking another machine. He hit steering wheel with mouth. This did not turn into a reportable until 10/08/04.
EE STATES WHILE CHECKING A HOSE BETWEEN FT-3 ANDTRAILER, FT-7 STRUCK SGAINST FT 3, CAUSING HIS KNEE TO BE STRUCK BY FT-3 BUMPER. INITIAL X-RAY DID NOT REVEAL ANY FRACTURES, ON 11-11 MRI RESULT S SHOWED FRACTURES.

WHILE WORKING BETWEEN BOOM TRUCK AND ROOF BOLTER BOOM TRUCK LURCHED FORWARD AND PINNED HIS LEFT LEG BETWEEN THE TWO PIECES OF EQUIPMENT.
EMPLOYEE STATES HE SAT DOWN TO REMOVE A ROCK FROM HIS BOOT. ANOTHER EMPLOYEE PULLED THE FORKTRUCK INTO THE CROSSCUT, PINNING THE EMPLOYEE'S FOOT BETWEEN THE BUMPER AND THE FLOOR.
EE STATES HE WAS OPERATING AN ALL TERRAIN VEHICLE (ATV) THE EE WAS UNABLE TO SLOW THE ATV DOWN WHEN DESCENDING A GRADE. WITH INCREASING SPEED HE DECIDED TO JUMP OFF THE ATV
