WHILE UNLOADING SUPPLIES FROM TRUCK BED, BULL HOSE WAS DROPPED, STRIKING TOP OF LEFT FOOT.

EE PULLING SHIELDS MANUALLY, RIGHT FOOT/ANKLE CAUGHT BETWEEN SHIELD AND PAN

EE was pulling in shields on LW face, fell while stepping to next shield striking posterior ribs/back.

EE was shoveling at a belt transfer, while in a crouching position to reach under belt EE felt a sharp pain in lower left abdomen which was diagnosed as a hernia.

EE donned Ocenco DBA 6.5 SCSR as part of routine training and fell unconscious after walking approximately 100 feet. EE was unconscious for approximately 45 seconds. Originally filed 7000-1 5/23/25

EE WAS CHANGING SHOCK ON BELT SCRAPER WHEN SCRAPER ARM FLIPPED UP AND CONTACTED LEFT INDEX FINGER. FINGER WAS AMPUTATED IN SURGERY AT 2ND KNUCKLE.

Employees were performing work tasks not focusing on other job tasks occurring around them. A group of employees were working on the lower scaffold deck directly below another group. One employee working directly above lost grip on the form ring, dropping their side allowing it to swing down knock their hardhat off and strike the employee in the back of the head.

Employee was running a come-along, EE thought the handle was locked in. When employee released the handle the tension brought handle back hitting employee in the safety glasses. Employee's safety glasses hit employee's eyebrow and cutting into the skin. Employee ended up getting 5 stitches.

Employee fell onto employee's proximity pad at the right hip resulting in contusion and bruising to the right hip. Employee was seen the same day and given a full release with no restrictions. During a follow up appointment the employee was placed on restrictions preventative in nature.

Employee was tramming bolter from ATRS controls. EE walked toward cable reel to check cable, returning to ATRS controls EE smacked knee on fire suppression bottle/bracket and dislocated knee cap.

Employee pulled on about 40ft of miner cable swinging it towards the rib with their right arm when they felt a "pop" in their right elbow and had discomfort. Suspected torn bicep.

employee was operating left boom of the roof bolter when a piece of roof mesh they were placing on the ATRS slid over the front catching fingers of both hands against the machine.

Individual operating water wagon pulled into crosscut out of the way of traffic when a drill steel came through the floor striking right knee.

Employee was removing a Kennedy panel from a stopping. As EE looked up to see what the top of the panel was caught on, a baseball size rock with sharp edges fell from the top striking EE in the face and lacerating EE's upper lip.

EE WAS CUTTING FLAP ON BELT WITH RETRACTABLE KNIFE, CUT LEFT HAND NEAR BASE OF THUMB WHILE TRYING TO CUT BELT. SUSTAINED LACERATION TO HAND WITH TENDON INJURY.

An accident occured and a miner tried to catch another miner who was falling. The miner has been on leave since the accident. The miner reported shoulder pain the following day and was diagnosed with a shoulder sprain several weeks later.

Riding in mantrip to work section hit a pothole bounced EE.

While using miner puller eyelet broke and came up and hit EE in the arm.

EMPLOYEE WAS ATTEMPTING TO CLEAN A CLOGGED SPRAY NOZZLE AND WAS SPLASHED IN THE EYE BY WATER AND DIRT. EMPLOYEES EYE WAS FLUSHED AND TAKEN TO HOSPITAL WHERE IT WAS CLEANED OUT BETTER AND EE WAS GIVEN A PRESCRIPTION MEDICATED EYE OINTMENT.

ANOTHER EE HEARD PANEL SLIDE ACROSS OTHERS, LOOKED AROUND STOPPING AND SAW INJURED WORKER ON THE GROUND. EE RETURNED TO NEXT SCHEDULED SHIFT, THEN SAW MD 3/13/24 FOR FOLLOW UP AND WAS PLACED OFF WORK FOR SYMPTOMS OF POSSIBLE HEAD INJURY.

roof fall found at ssms return 3E, 33xc intersection. The roof fall is approx. 10-12 feet at its highest point which is above anchor point. It is approx. 30 feet long by 23 feet wide. The affected area is flagged off.

WHILE CUTTING COAL ON THE LONGWALL, THE SHEARER SLOUGHING PLATE CAUGHT A SPRAY BAR HOSE CAUSING IT TO STRETCH TIGHT BEFORE DISLODGING AND WHIPPING BACK TOWARD THE OPERATOR. THE BAR STRUCK THE EMPLOYEE ACROSS THE LEFT SIDE OF THE HEAD AND NECK, RESULTING IN A CRUSHING LACERATION TO THE LEFT EAR WHICH REQUIRED 22 SUTURES.

Twisted ankle while shoveling accumulations off the top of the miner.

THE EE CLIMBED INTO A SHIELD WAITING FOR THE SHEARER TO PASS. WHEN THE SHEARER PASSED, THE EE CLIMBED OUT OF THE SHIELD, STEPPING ON THE NEXT SHIELD THAT HAD JUST PULLED, THE FLIPPER ON THE SHIELD TIP HAD NOT YET SWUNG OUT INTO POSITION. THE FACE SLOUGHED OUT ACROSS THE PANLINE AND BRETBY TROUGH, MATERIAL STRUCK EE IN LEFT LOWER LEG.

While re-bolting a steep area where the screen was bagged, the employee grabbed the pan rack with right hand to help climb onto bolting platform. At that moment, a rock fell through screen striking the right middle finger about half way up the fingernail, resulting in the fingertip being amputated, and a tuft fracture of the distal phalanx.

MSHA Inspector and supervisor were walking weekly route and discovered a reportable roof fall in ESM, 8E, 21XC intersection that impeded the travel route for the weekly. Ventilation was not affected. The roof fall measured approximately 19'x21'x15 feet high. The area was timbered and dangered off and will be rehabbed in the near future.

Reinjured an existing injury to EE's left foot fracturing the fourth metatarsal. Injury was due to excess body weight on the employee. Employee stumbled catching self and applying all weight to the one foot.

Poor lifting technique in trying to lift too much alone. Should have gotten another person for help.

EE WAS HELPING REPOSITION THE BELT TENSIONER STATIONARY ROLLER ASSEMBLY AFTER IT BROKE LOOSE FROM ITS MOUNTING BEAMS. THE ASSEMBLY HAD BEEN STOOD UP AND CREW WAS GETTING READY TO REPOSITION ROPES TO MOVE ASSEMBLY FINAL POSITION. EES WERE BELIVED TO HAVE WENT IN TO MOVE ROPES WHEN THE ASSEMBLY FELL OVER STRIKING THEM.

EE WAS HELPING TO REPOSTION THE BELT TENSIONER STATIONARY ROLLER ASSEMBLY AFTER IT HAD BROKE LOOSE FROM ITS MOUNTING BEAMS. THE ASSEMBLY HAD BEEN STOOD UP AND THE CREW WAS GETTING READY TO REPOSITION ROPES TO MOVE ASSEMBLY INTO FINAL POSITION. EE IS BELIEVED TO HAVE WENT IN TO MOVE ROPES WHEN THE ASSEMBLY FELL OVER PINNING THEM.

Went to step out of operator's seat and foot slipped and employee hit the back of the operator's seat with mouth and chipped a front tooth.

While removing a DA cylinder from shield a come-a-long was being used to suspend the cylinder. When the electro-hydraulic function was cycled to move the cylinder it caused the come-a-long to spin striking the miner in the face with the handle.

While repositioning a bad relay bar for removal from the face the miner disconnected the safety chain and had another miner remove the come-a-long, when they did this the relay bar slipped off the shearer sliding down the other relay bars pinning miner's right leg against a relay bar resulting in a fracture to the right tibia.

Employee lifted 72" return roller into back of pickup and felt pain in right shoulder and arm. Employee continued to work until surgery on 2/3/2021.

MINER WAS HANDLING END CAP OF OVERCAST. AS EE WAS LIFTING IT TO PLACE IT ONTO PANELS, MINER SET IT DOWN ON LEFT LITTLE FINGER CAUSING A LACERATION AND FRACTURE THAT REQUIRED SURGERY TO SET.

While transferring rock dust, the line blew apart striking the miner on the right side of the face causing a laceration requiring sutures to close.

Injured miner was working on a belt splice with two others employees setting a row of rivets, they had completed rivets and was backing away with head bowed down to avoid obstacles. One of the other employees struck a rivet and a piece of metal broke off and struck the injured miner in the left eye over the top of safety glasses. Resulting in a contusion to the eye.

Weekly examiner heard the roof fall from 2 breaks away and went to investigate. Found fall SS1TG, E1, xc6-7 approx. 10'h x 18'w x 30'l approx. 2' above anchorage. This fall impeded travel but did not damage ventilation.

Hoist lost communication with AMS. Upon investigation, it was found to have tripped power and would not reset the hoist safety system. The safety system was reset and the hoist became operational at 4:49 pm.

While recovering an I-beam, the beam came loose and struck the miner landing on right leg. The injury is soft tissue damage. X-rays were negative for broken bones.

Poorly laminated shale roof, with slicken sides and coal riders throughout. Also, undermining of previously mined B-seam LW panels.

Employee inadvertently walked between scoop bucket and rib and was struck when scoop moved.

A reportable roof fall was found in Sunset South 1 Headgate, entry 1, crosscut 22. The fall was approximately 25' long, 19' wide and 10' high.

While roof bolting, employee reached underneath the drill steel that was still in the drill hole. The drill steel fell, striking EE's left index finger, resulting in a partial amputation.

Employee was determined to have a standard threshold shift based on audiogram.

WHILE LIFTING PUMP, EMPLOYEE'S LEFT RING FINGER WAS PINCHED BETWEEN THE PUMP AND AN ADJACENT PUMP OR STORAGE TUB RESULTING IN A LACERATION.

Tripped while walking between two components. Fell forward and hit head on a truck, causing a laceration to EE's forehead.

Employee stepped off of bolter to ground below rolling ankle on a piece of coal.

While moving roof bolter cable, the rib sluffed off striking the employee.

Escape way hoist in Shaft 1 was found to be inoperable at 3:11 a.m. A line filter had shorted out on the hoist safety system. The hoist was returned to service at 3:45 a.m.

Employee was hanging vent tube when they stepped onto stabilizer bar and another employee lowered the boom which resulted in a structural member contacting the top of the foot which resulted in dislocating the four small toe bones.

Employee was attempting to remove a jackleg hammer from the back of a flatbed truck. The employee stood the hammer upright on the chuck bail. To remove the hammer, employee placed one hand under the bail & grabbed the hammer body with the other hand. The bail collapsed causing the hammer to fall, pinching employee's little finger, resulting in a fractured finger.

Upon daily hoist inspection, the hoist computer system was malfunctioning which prevented the operation of the hoist. It was determined that the battery backup caused the problem. The battery backup was disconnected and hoist functions returned. The hoist was not repaired within 30 minutes.

Employee was working on seal when rib section rolled out. Unable to avoid the rib and was struck in the lower back.

Employee was installing a spot roof bolt. While drilling the hole, a piece of rock fell around the drill steel, striking the employee's forehead and causing a skull fracture and laceration

An employee was standing on a Ramcar, washing it off, when another employee started to move a nearby Ramcar in the opposite direction. The moving Ramcar caught the roof bolter cable hanging from bullhorns, pulling the cable down. The moving cable forcibly struck the employee standing on the Ramcar, knocking that employee down on the Ramcar.

During the weekly examination, a reportable roof fall was found in the established travelway. The fall occurred in F Seam, South Main Intakes, entry 2, crosscut 17. The fall is estimated to be 20' wide, 20' long, and 6' high.

EE was operating shearer when a piece of coal fell into the bretby trough. EE stopped the shearer from tramming & removed the piece of coal. When returning the shearer, another piece of coal fell from longwall face, striking EE, pushing EE into the shield base lift cylinder. Reportable medical treatment was not provided until March 8, 2017. Became lost time 4/26/17.

Employee was installing roof support cans, the single head axe that EE was using to tighten the wedges caught a string line as EE swung the axe which caused the axe to jerk sideways striking the employee in the face. The employee received lacerations to the upper lip, lower lip, and upper gum which required stitches. One front tooth was also partially dislodged.

Employee states EE was retrieving a water hose that was under a jack leg hammer. When the hose was moved, the hammer fell catching hand between the hammer and shield control unit.

Piston seal on hoist brake caliper failed preventing brake from fully functioning, resulting in the hoist being down for more than 30 minutes.

A reportable roof fall was found in E East Mains, Entry 1, crosscut 12. The fall approximately 6.5' high, 50' long, and 18' wide.

A reportable roof fall was found by pre-shift examiner in E7 Headgate, entry 2, crosscut 31. The fall was approximately 20' long, 22' wide, and 9' high.

During weekly examination, a reportable roof fall was found in E12 Rock Rooms, entry 10, crosscut 2 to 3. The fall was approximately 20' in length, 16' wide and 9' high.

While riding in the back of a mantrip on a road that had been excessively watered, the employee struck against the mantrip shell when EE's vehicle slid into another mantrip that was stopped in the entry, resulting in back pain and a sprained ankle.

During the weekly examination, a reportable roof fall was found in F Seam Southwest Main Intakes in entry 3, XC-20. The fall was approximately 30' long, 15' wide and 7' high.

A reportable roof fall was found during the weekly examination in Sly Gulch Return, Entry 3, crosscut 10. The fall was approximately 45' long, 27' wide and 7' high.

After delivering concrete to mine, employee's foot slipped on the combined sand & concrete build-up that was around the pipe that the employee was pouring concrete into, causing him to fall. When falling he stretched out his right arm & hand to catch himself & his right shoulder became dislocated when his hand hit the ground.

Employee was trying to pull a bent roof bolt back out of the roof. He hooked the roof bolt plate with the mast, the plate slipped and swung back, striking the employee on the lip.

While performing the daily hoist inspection, the power tripped due to a fault in the process logic system. The hoist was down approximately 83 minutes while the problem was diagnosed and the faulty positioning switch was replaced.

As employee was lifting top frame of 84" structure on to a pallet, he started to feel pain in his groin area.

A reportable roof fall occurred in E South Mains, Entry 3 at crosscut 19. The fall was approximately 15 feet high, 25 feet wide and 25 feet long, damaging the high voltage power cable and nitrogen line.

EMPLOYEE STATES HE WAS USING A PRY BAR TO UP-RIGHT AND ALIGN THE BALDERSON ATTACHMENT WITH THE FORKS ON A SCOOP. HE PRIED DOWN ON THE BAR, UP-RIGHTING THE ATTACHMENT. ONCE UP-RIGHTED, THE ATTACHMENT TIPPED FORWARD CAUSING THE PRY BAR TO STRIKE THE FRAME OF THE FORKS. THE PRY BAR RAPIDLY SPRANG UPWARD, STRIKING THE EMPLOYEE IN THE HEAD, RESULTING IN A LACERATION.

AMS operator discovered that Deer Creek hoist had lost operational power. Due to surface travel distance to the site, the hoist was inoperable for more than 30 minutes. When personnel arrived at the site, they found a circuit breaker had tripped. The circuit breaker was reset, restoring power to the hoist.

Employee states he and another employee were positioned in the cable basket removing de-energized high voltage cable. After removing a strap, the cable lodged between the angled support bolt and the stirrup on a truss. When the next strap was removed, the cable dislodged from the truss, striking employee's hardhat and pushing his head into the cable basket causing a laceration.

EMPLOYEE STATES HE WAS INVESTIGATING WHAT WOULD BE NEEDED TO PULL OUT A PLOW TRUCK THAT WAS STUCK IN THE SNOW. HE WAS WORKING HIS WAY DOWN THE SIDE OF THE VEHICLE, HOLDING ON TO THE SIDES, WHEN BOTH HIS FEET SLIPPED OUT FROM UNDER HIM. HE FELL TO THE GROUND, LANDING ON HIS RADIO WITH HIS LEFT LEG, MID-THIGH, RESULTING IN A FEMUR FRACTURE.

Employee picked up a bottle of oxygen off the welder trailer, slipped and fell and hit his back against the hitch on the trailer.

EMPLOYEE WAS INSTALLING A TRUSS BOLT. HE ATTEMPTED TO PULL A STUCK STEEL BY GRABBING THE BOTTOM OF THE DRILL STEEL. THE STEEL FELL, CAUSING HIM TO PINCH HIS LITTLE FINGER BETWEEN THE DRILL STEEL AND THE JAW CLAMP, RESULTING IN A LACERATION REQUIRING SUTURES.

ON 9/15/2014, EMPLOYEE STATED HE HAD RECEIVED NOTIFICATION FROM MSHA THAT HE WAS ELIGIBLE TO EXERCISE PART 90 MINER RIGHTS.

Driving a DF-54, when he hit a slick spot in the road causing the DF-54 to hit the rib.

While conducting the daily hoist inspection, the hoist safety supervisor system failed, resulting in the hoist being out of service for more than 30 minutes. The hoist was returned to service at approximately 12:20 p.m..

While traveling with an MSHA inspector on a weekly examination route, a reportable roof fall was found in F Seam, Southwest Main Intakes, entry 3, at crosscut 22. The fall was partially located in inactive works, but affected the established travel route for the weekly examiner. The fall was approximately 6' high, 18' wide, and 38' in length.

EMPLOYEE STATES AS HE WAS WALKING TO MANTRIP HE STEPPED ON AN UNKNOWN OBJECT, STRAINING HIS RIGHT KNEE WHICH RESULTED IN A TORN ACL / MCL.

A reportable roof fall was found in E East Mains, entry 3, crosscut 6. The roof fall was approximately 35' long, 21' wide, and 9.5' high.

A roof fall occurred in inactive works that damaged an adjacent stopping in active works, resulting in impaired F-Seam ventilation. The fall occurred in F Seam, South Main Returns, entry 6, between crosscuts 14 and 15. The fall was approximately 5' high, 30' long, and 18' wide.

Operator was working to unbridge #26/27 feeders on R-5 Reclaim. He was working the south side of the feeders, pushing toward #24/25 feeders. He was backing up for another push, when he misjudged his location and got too close to the bridged #26/27 feeders. As a result the dozer broke through and fell into the #26/27 feeders.

Employee experienced sharp pain in sacrum area and numbness in toe/foot after pulverizing. This claim is being disputed. It may be related to a previous injury.

A reportable roof fall was found in E South Mains, entry 6, crosscut 28. The fall was approximately 10' high, 20' wide and 25' long.

Hoist became inoperable at 8:15 p.m.. Upon investigation it was found that switch synchronization would not occur properly. Programming was corrected making the hoist fully operational at 11:45 p.m.

Employee alleges that he has occupational asthma as the result of being exposed to dust and fumes in the working section.

A power surge on the feed line tripped circuit breakers to the hoist. The main external circuit breaker was reset but not the internal circuit breaker. A qualified person was sent from the mine site to reset the internal circuit breaker but due to travel time, the hoist was down for more than 30 minutes. When the internal breaker was reset at 10:10, the hoist was fully functional.

A roof fall occurred in inactive workings in F-Seam, entry 7, crosscut 14 to 15 in the Southwest Main Returns. The roof fall in inactive works partially damaged an adjacent stopping.

A reportable roof fall was found in E4 Headgate, entry 1, crosscut 19 to 20. The fall was approximately 10 to 12 feet high, 20 feet wide and 30 feet long.

A reportable roof fall occurred at F-Seam, #71 XC, between entries 5 and 4. The fall was approximately 17' wide, by 20' to 25' long, by 6.5' to 7' high.

While operator was conducting a shaft inspection from the hoist inspection deck, a tree struck the surface power line causing an underground power outage. The person performing the shaft inspection was unable to leave the hoist until power was restored approximately 45 minutes later.

A reportable roof fall was found in E Seam, E4 Headgate, entry 1, between crosscuts 20 and 21. The cap rock failure prevented access through the belt entry. The cap rock failure was approximately 20 feet wide and approximately 13 feet in length.

Employee was cleaning behind a door. Another employee entered the room carrying a heavy box. The door swung and the door handle hit employee in the lower back. Employee worked through the shift and the next day, went to a Dr. 2 days after who stated it was bruised but problem was degenerative. Worked through 08/24/2012 when another Dr. said pain was from incident.

Riding in the back of a mantrip at the end of his shift when EE hit his head against the top of the mantrip jamming his neck when the mantrip hit a bump.

EE states he had hand on hydraulic boom and leaned over fork truck to check position of hitch. Another employee rolled the forks back, catching EE's left ring finger between the barrel and the eye of the hydraulic cylinder, resulting in a laceration requiring sutures.

While performing system checks on the automatic hoist at Shaft #1, a programming glitch occurred, making the hoist inoperable. The programming glitch was identified and corrected, allowing the hoist to be put back into immediate operation after being inoperable for approximately 50 minutes.

A reportable roof fall was found in F Seam, entry 5, between crosscuts 25 and 26. The fall was approximately 20' long, 18' wide, and 6.5' high.

EMPLOYEE STATES THAT AS HE WAS CUTTING A PIECE OF WOOD WITH AN AX, HE STRUCK HIS LEFT INDEX FINGER, RESULTING IN A LACERATION REQUIRING SUTURES.

EMPLOYEE WAS INSTALLING A CLAMP ON A STEEL CABLE. THE NUT WAS STARTED ON THE U-BOLT. INDIVIDUAL RELEASED HIS HOLD AND REACHED FOR A WRENCH TO COMPLETE TIGHTENING THE NUT. NUT CAME OFF, RELEASING THE U-BOLT WHICH STRUCK THE EMPLOYEE, CHIPPING HIS UPPER FRONT TEETH. EMPLOYEE DID NOT VISIT THE DENTIST UNTIL 03/16/2012 AT WHICH TIME HE HAD THE TEETH REPAIRED.

When advancing tailgate shields during normal mining, the immediate roof fell in front of the last four shields, preventing access to the tailgate entry from the longwall face.

Employee alleges he was operating the tailgate shearer drum as the shear cut toward the tailgate. When the face popped ejecting a piece of coal approximately 8 X 4 X 2 striking employee on the left side of the head resulting in a laceration to the ear and to the side of the head requiring stitches.

EMPLOYEE STATES HE WAS CUTTING TABS OFF A SCREEN PANEL USING A UTILITY KNIFE WHEN KNIFE SLIPPED, RESULTING IN A LACERATION TO HIS LEFT RING FINGER REQUIRING SUTURES.

EE's hand was on the rail when a piece of coal hit his hand smashing his index finger.

PRINCIPAL STATES HE WAS RAISING A 20' PIECE OF 6" STEEL PIPE ONTO BED OF BOOM TRUCK WHEN PIPE BECAME UNHOOKED, FALLING TO THE GROUND AND STRIKING HIS RIGHT ANKLE, RESULTING IN A LACERATION REQUIRING SUTURES.

While cutting towards the headgate with the longwall shearer, sparks from the cutting drum apparently ignited hydrocarbons coming from the roof resulting in a fire. The fire was extinguished in approximately 20 seconds by the fire suppression system and a water hose.

EMPLOYEE STATES THAT AS HE WAS PLACING A CRIB BLOCK UNDER THE EMULSION TANK, HIS FINGER CAUGHT BETWEEN THE BLOCK AND THE TANK, RESULTING IN AN AVULSION TO THE TIP OF HIS LEFT INDEX FINGER.

Moving caps, wedges and crib block though man door at 3 entry 71 xc E-3 HG, when he felt pain in his stomach that got worse while moving this material.

EE states he was checking sample system. As he opened door on sampling system, the cutter activated pushing employee's head against door frame, resulting in a laceration to the left ear which required stitches.

EE STATES AS HE WAS INSTALLING ROOF BOLT, HE HAD TO REACH IN BETWEEN THE SCREEN AND ROOF TO GET HIS GLUE INTO THE HOLE. AS HE REACHED UP, A PIECE OF ROCK FELL FROM THE ROOF, STRIKING HIS LEFT FOREARM, RESULTING IN A LACERATION REQUIRING STAPLE.

Lab Tech was crushing a sample. The buckets were heavier than usual, a 40# bucket of coal crushed into a 21# pan made for a 60# lift. Employee felt something in her abdomen 'pop'. On further investigation found a protrusion. Employee was taken to local hospital and diagnosed with urethral prolapse. This injury requires surgery which is scheduled for Wednesday, March 2 at 1:30 PM.

A reportable roof fall was found in E2 Headgate, entry 1, at crosscut 60. The fall was approximately 16' wide, 35' long, and up to 9' high.

EMPLOYEE STATES HE WAS LIFTING A PILLOW BLOCK BEARING CAP WHEN HIS GRIP SLIPPED, PULLING HIS RIGHT HAND DOWN. RIGHT RING FINGER WAS CAUGHT BETWEEN BEARING CAP AND BEARING SURFACE, RESULTING IN A LACERATION REQUIRING SIX SUTURES.

SHEARER STRUCK SPRAG BREAKING A COVER PLATE LOOSE, STRIKING EMPLOYEE ON THE RIGHT FOOT, RESULTING IN A FRACTURE. FRACTURE WAS NOT DIAGNOSED UNTIL 01/19/2011. FIRST DAY MISSED WILL BE 01/21/2011.

EMPLOYEE STATES WHILE OPERATING TG END OF SHEARER, COAL DUST ENTERED EYE, RESULTING IN PHYSICIAN INTERVENTION TO HAVE A PIECE OF COAL REMOVED FROM CORNER OF RIGHT EYE.

EMPLOYEE STATES HE WAS REPAIRING ACTUATOR ON DUST SYSTEM WHEN ACTUATOR BECAME ACTIVATED, CATCHING HIS FINGERS, RESULTING IN A PARTIAL AMPUTATION OF LEFT MIDDLE FINGER AND FRACTURE OF RING FINGER.

Bottle of cleaning solution had damage to screw top lid. Bottle slipped from employee's hand to the floor and solution splashed up into employee's face and eye.

Pouring concrete for pad to put belt components on, where concrete worked into eyelets on leather boots over a period of time causing burns.

During the weekly examination, a reporatable roof fall was found in E10 Headgate, entry 2, crosscut 2. The fall was approximately 20 feet wide, 20 feet long and 12 feet high.

Employee was kneeling and stood up. Stepped on harness lanyard. Fell forward hitting left knee.

When longwall tailgate shields were lowered for advancement, roof rock fell from above the shields, resulting in a tailgate blockage. Blockage occurred in entry 3, inby crosscut 53, E1 Headgate.

EMPLOYEE STATES SHE WAS LOADING AIR PUMP ONTO BACK OF TRUCK WHEN SHE LOST HER FOOTING. SHE FELL TO THE GROUND AND THE AIR PUMP LANDED ON TOP OF HER RIGHT LITTLE FINGER.

EE was trying to get a cap off conveyor and when the binding came free he smashed his left hand, middle finger and it required 5 stitches. There is no lost time on this. He was released back to work with no restrictions. One follow up appointment to remove stitches.

A reportable roof fall was found in E South Mains, entry 2, at crosscut 27. The fall was approximately 20 feet wide, 20 feet long, and 6 to 8 feet high.

Employee states he was welding on a refuse bin and slipped in the mud when relocating the bin. The employee jammed his right thumb as he was attempting to stop his fall, hyper extending his thumb. Employee sought medical treatment and worked 5 days on modified duty.

Employee states while prying up on stater housing to the Tek-Seal pump with a pick, the pick slipped causing employee to fall on his right wrist, resulting in a fracture.

In process of swinging trench roller with trackhoe into trench, employee went to hook roller to bucket. While doing so, co-worker hit lever which rolled machine onto employee's foot.

WHILE ROOF BOLTING, EMPLOYEE WAS ADDING EXTENSION STEEL TO STARTER STEEL WHEN OPERATOR'S PLATFORM SHIFTED, CAUSING HIM TO LOSE HIS BALANCE. IN AN EFFORT TO CATCH HIMSELF, HE LET GO OF STARTER STEEL WHICH FELL FROM ROOF, STRIKING HIS LEFT RING FINGER RESULTING IN A FRACTURE AND LACERATON.

EMPLOYEE WAS ATTEMPTING TO ENTER THE CAB OF THE BOBCAT LHD. WHILE TWISTING TO SIT ON THE SEAT, HE FELT BACK PAIN.

During the weekly examination, a reportable roof fall was found in F-Seam, entry 4 at crosscut 20. The fall was approximately 20 feet wide, 20 feet long, 6 feet high.

EMPLOYEE APPARENTLY THOUGHT ELECTRICAL POWER HAD BEEN DISCONNECTED AND WAS USING TWO SCREWDRIVERS TO DISSIPATE ANY REMAINING CHARGE. THE SCREWDRIVERS CONTACTED ENERGIZED COMPONENTS, RESULTING IN SECOND DEGREE FLASH BURNS TO BOTH HANDS.

Two employees were using the Deer Creek escapeway hoist when the motor tachometer malfunctioned, resulting in an overspeed fault, stopping the hoist. Due to the remote location of the shaft, the employees were caught on the hoist for approximately 45 minutes until the fault could be cleared from the surface control panel.

A reportable roof fall occurred in E1 Headgate, entry 1, between crosscut 25 and 26. The fall was approximately 32' long, 18' wide and 8' high.

During the weekly examination, a reportable roof fall was found in F-Seam, entry 7 at crosscut 78. The fall was approximately 18 feet wide, 13 feet long and 8 feet high.

Employee states he was attempting to replace cover plate on petitto mule when cover plate slipped, striking employee on left foot.

Employee states he pushed vent tubing closer to roof with one hand. He pulled strap tight with the other. When he jerked on strap, he twisted, resulting in lower back pain. He continued to work until November 3, 2009, at which time an MRI identified a pinched nerve in his back. Doctor removed employee from work to allow treatment for the condition.

EMPLOYEE STATES SHE HAS PAIN IN LEFT THUMB, HAND AND ARM FROM REPETITIVE USE ON COMPUTER. SURGERY TO THUMB OCCURRED ON 01/20/2010.

EMPLOYEE STATES HE USED A JACKLEG HOSE TO PULL HIMSELF UP FROM A KNEELING POSISITON, CAUSING THE JACKELG TO FALL AND STRIKE EMPLOYEE'S THUMB, RESULTING IN A RIGHT THUMB FRACTURE AND LACERATION.

Hoist would not operate due to an encoder failure.

EE was scaling rock when a rock fell striking him on the head and shoulder causing a fractured collar bone and a possible compression fracture.

A reportable roof fall was found in entry 4, crosscut 4 of F South Mains. The fall was estimated to be approximately 50' long, 30' wide, and 16' feet high.

EE was removing loose rock in wire mesh when a rock fell striking his right 5th finger causing it to hit the man basket. EE received a laceration that required stitches.

While spinning a cable bolt to mix the resin, the plate was in contact with the ray mat, resulting in a frictional ignition of methane. The small flame was immediately extinguished with a fire extinguisher.

EE WAS REMOVING SNOW OFF THE LORAIN CRANE WHEN HE SLIPPED OFF THE DECK AND LANDED ON CONCRETE SHAFT COLLAR CAUSING CONTUSION TO TAILBONE.

EE tried to catch a falling block.

EMPLOYEE STATES WHILE LOWERING ATRS ON ROOF BOLTER, IT CAUGHT ON A MONKEY FACE, PUSHING THE MONKEY FACE INTO THE EMPLOYEE'S HAND AND ARM, CAUSING A FRACTURE TO TIP OF FINGER AND LACERATION TO ELBOW.

During the weekly examination, a reportable roof fall was found in F-Seam Sly Gulch return, entry 7 to 8, crosscut 16. The fall was approximately 25' long, 12' wide and 7' high.

EE was installing a sleeve on the wing of the raise bore head bolt, when his left 5th finger was smashed into the wedge pin, causing a laceration that required stitches.

During the weekly examination, a reportable roof fall was found in Box Canyon Mains, entry 8, crosscut 0. The fall was approximately 20 feet long, 20 feet wide and 8 feet high.

A reportable roof fall was found in F-Seam, entry 5, crosscut 72. The fall was approximately 40' long, 19' wide, and 8' high.

During the weekly examination, a reportable roof fall was found in F-Seam, Southwest Main Intakes, entry 3, crosscut 29. The fall was approximately 18' wide, 18' long, and 7' high.

Employee was capping top of can, when he stepped down off ladder he stepped on piece of wood rolling his ankle.

EMPLOYEE STATES HE WAS WALKING, CARRYING A PIECE OF BRATTICE. HE TRIPPED ON A PIECE OF COAL AND PUT HIS ARM OUT TO CATCH HIMSELF AS HE FELL, STRAINING HIS LEFT SHOULDER. RECORDABILITY DID NOT OCCUR UNTIL 01/16/2009.

A reportable roof fall was found in E-1 Headgate, entry 1, crosscut 26. The fall was approximately 25 feet in length, 25 feet wide and 8 feet high.

EE STATES HE WAS ROCK DUSTING THE BELT ENTRY USING SV-5 WHEN THE VEHICLE GOT STUCK.TO BACK THE VEHICLE UP,HE TURNED AROUND IN OPERATOR'S SEAT & PLACED HIS RIGHT HAND ON TOP OF THE FUEL TANK BEHIND HIM.WHEN VEHICLE MOVED,IT SLID TOWARDS BELT STRUCTURE,CATCHING HIS RIGHT INDEX FINGER ON A RETURN BELT ROLLER HANGER, RESULTING IN A LACERATION THAT REQUIRED SUTURES.

EMPLOYEE STATES HE RELEASED BINDER HOLDING CANS ON TRAILER. TENSION ON CHAIN BECAME TOO GREAT, CAUSING BAR TO STRIKE HIM, RESULTING IN A CHIPPED TOOTH.

While traveling the return entry, a mine examiner found a reportable roof fall in E-1 Headgate, entry 3, XC 41 to 42. The fall was approximately 6 feet high, 20 feet wide, and 25 feet long.

Employee states he was raising belt structure. He was the tallest. As it was brought down, the weight transferred to him. C channel on side rail came in contact w/ hard hat and he felt a pop in his back. Lost time for this incident did not begin until 09/20/2008.

A reportable roof fall occurred in E-1 Headgate, entry 2, crosscut 47. The fall was approximately 12 feet high, 20 feet long, and 10 feet wide.

Employee was holding a rock dust bag above his head as a strength prank. Co-worker reached behind him and cut bag with a knife and cut his thumb.

Helping another person remove some cap rock off of a power cable, while he was bent over a piece of cap rock fell out of void striking him on the back of the hard hat.

EMPLOYEE STATES HE WAS OPERATING THE CONTINUOUS MINER WHEN A RIB BLEW OUT, STRIKING EMPLOYEE IN THE MOUTH.

During the weekly examination a roof fall was found at E-1 Headgate section #3 entry between crosscuts 25 to 26. The fall was approximately 40' long, 5' to 6.5' high 14' to 16' wide.

EMPLOYEE STATES HE WAS IN FORK TRUCK BASKET INSTALLING CRIB. AS BASKET WAS LOWERED, EMPLOYEE HIT RAIL ON BASKET, STRIKING HIS LEFT WRIST ON RAIL, CAUSING A LEFT WRIST FRACTURE.

EMPLOYEE STATES HE WAS CUTTING OUT A PIECE OF DUST LINE. THE HOSE WAS LAYING ACROSS HIS RIGHT LEG. AS HE CUT THROUGH THE HOSE, THE KNIFE TURNED AND CUT EMPLOYEE'S RIGHT LEG. EMPLOYEE LATER WENT TO THE HOSPITAL TO HAVE STAPLES; HOWEVER, HE DID NOT INFORM MANAGEMENT OF THIS UNTIL 11/06/2008.

Was operating a fork truck, as he went around a corner the rear tire caught a drill steel that was leaning aginst the rib pulling it forward striking him in the hard hat. The hard hat was forced down and caused a laceration to the L-side of his head.

He was carrying a 16' x 5' roof screen along longwall face on recovery when he tripped and fell hitting his right ear on another screen that was laying there.

EMPLOYEE STATES HE WAS REACHING FOR MATERIALS WHEN A PIECE OF COAL RIB BLEW OUT, STRIKING EMPLOYEE ON LEFT SIDE RESULTING IN LEFT WRIST DISLOCATION AND LACERATIONS TO THE FACE AND ABOVE LEFT ELBOW.

EMPLOYEE STATES HE WAS WALKING WHEN HE STEPPED THROUGH SOME WATER, CAUSING HIM TO STUMBLE, HYPEREXTENDING HIS RIGHT KNEE.

Employee found a reportable roof fall in E-1 Headgate, entry 3, crosscut 27. The fall was approximately 22 feet wide, 25 feet long, and 6 feet high.

EMPLOYEE STATES AS HE WAS BOLTING, THE PUSHER STEEL CAUGHT ON THE WRENCH AND BENT THE STEEL INTO THE EMPLOYEE'S LEFT WRIST.

Employee was building a stopping, after foaming he stepped down on a chunk of coal rolling his foot and ankle causing a fracture to the bone on the small toe.
