TWO EMPLOYEES WERE PUTTING AWAY AN OXYGEN CYLINDER, TRANSPORTING THE BOTTLE TO THE STARAGE AREA. AS ONE EMPLOYEE PICKED UP THE CYLINDER IT SLIPPED OUT OF HIS HAND & STRUCK THE TRAM PEDAL. THE VEHICLE MOVED & STRUCK THE OTHER'S LEG. CAUSE OFINJURY WAS NOT SHUTTING OVER POWER TO EQUIPMENT & SETTING BRAKE. COMPLIANCE WITH RULES/REGS WAS A FACTOR. MINER PROFICIENCY A FACTOR.

HE STEPPED BETWEEN THE MOVING RAM AND THE SHIELDCAR. CAUSE OF INJURY WAS COMMUNICATION AND AWARENESS. COMPLIANCE WITH RULES/ REGS WAS A FACTOR, MINER PROFICIENCYA FACTOR,
EE WAS USING A SCOOP TO CLEAR GOB, A PIECE OF RIB, STONE/COAL MIX FELL OUT STRIKING CANOPY. A PIECE CAME IN CAB STRIKING VICTIM ON BACK. CAUSE OF INJURY WAS AWARENESS & KNOWLEDGE. COMPLIANCE W ITH RULES/REGS NOT A FACTOR, MINE EQUIP/SYSTEMS NOT A FACTOR, JOB SKILLS AND MINER PROFICIENCY MAY BE A FACTOR. PROTECTIVE DEVICES ON EQUIPMENT A FACTOR.
