Employee was found unresponsive sitting in a seat of a skid steer at 15:24pm the investigation found nothing that contributed to this death it is believed that the employee died of natural causes.

Employee was resetting post when a piece of rock struck EE on the lip resulting in a laceration. Cause of this incident was lack of employee awareness. Employee was handling belt clamp when one side dropped causing EE to catch EE's finger. Cause of this incident was lack of employee concentration. Employee was unloading material from scoop car onto supply cars, doing so EE tripped resulting in a sprained wrist while catching self. Cause of this incident was lack of EE concentration. EE was using handle on coupler to unhook motor from shield car, due to slack in the rope the cars inadvertently drifted ahead coming into contact with EE left foot. Employee was lifting battery lid on scoop when EE felt a pain in the right shoulder. Employee continued working until EE underwent surgery on 4/25/2019 becoming a RLT. While helping another employee who had fallen, the injured employee fell and struck knee against a block, resulting in a laceration.

Employee was installing rock duster onto scoop bucket when finger was caught between a header board and the rock duster. Cause of this incident was employee concentration. EE was aligning the coupler by use of the handle from inside the goodman motor when they felt a pop in their biceps resulting in a tear. Cause of this incident was employee concentration. EE was removing cover from tailgate drive motor when a rock struck EE in the back of the legs causing EE to fall forward striking cheek on the tailgate drive motor frame work. Employee was lowering a track jack when EE's finger was caught between the cap of the jack and the body of the jack resulting in a laceration. Cause of this incident was employee concentration. Employee was operating the continuous miner when a piece of foreign debris scratched EE's eye resulting in medical eye drops. Cause of this incident was employee knowledge. While employee was relieving pressure on duke jack, jack became dislodged catching employee's thumb between spill tray and jack. Lack of employee concentration was the cause of this incident. EE and co-worker were moving a peace of rail, co-worker tossed their end landing on EE's foot fracturing bones in top of foot. Cause of incident was lack of communication between employees. While removing a screw jack from underneath a shield canopy, employee placed hand between the crib block capping the jack and the shield canopy. With hand in this position, the shield depressurized, pinning EE's hand between the shield canopy and the crib block, causing a fracture and laceration to the right ring finger requiring stitches.

EE was placing a beam on top of next line shield to be recovered. The beam came in contact with above roof dislodging a rock. Rock struck EE on top left forearm causing laceration. EE was being task trained on man lift. While moving it off the trailer a wood board broke on the trailer decking. This caused the machine to jerk and ee hit elbow on the basket railing. EE was setting up for a belt move, doing so ee turned while carrying a bottom roller resulting in a strain/sprain to ee lower back. Cause of this incident was employee concentration. Employee was installing water manifold on head gate water tree, while doing so EE was struck in the finger with a hammer resulting in a laceration requiring glue. Contract employee was helping another contractor to change a roll of felt material, cardboard tube got caught on roller and while using a knife, contract employee accidentally cut finger. EE was traveling on rubber tire personal carrier when a mechanical failure was incurred resulting in the employee losing control striking the ground with shoulder. Mechanical failure was the cause of this incident. Employee was using utility knife to remove burrs from plastic pipe, doing so the knife slipped causing a laceration to the hand resulting in stitches. While employee was opening a man door was struck in the eye by foreign body item resulting in a scratch. Cause of this incident was minute flying material. Contract employee was cutting the belt for making a mechanical splice, as EE was cutting the belt, the knife contacted upper leg when EE reached the edge of the belt resulting in a laceration. EE was fueling heaters on 3/12/17 in prep plant, EE was struck in the hard hat by rust scale from overhead beam. EE worked w/o restriction until seeking med attn. on 3/17/17 becoming RLT. Employee was tramming scoop down entry, while turning into x-cut employee caught fingers between stop block and operators kitchen. Cause of the incident was lack of employee concentration. Employee was changing bottom belt roller along main line belt. While installing new roller employee dropped roller onto left ankle resulting in a fracture. EE was pulling shields on longwall face, EE turned to move away from a spraying hose and caught foot between a relay bar and shield putting stress on and fracturing right ankle. EE was using 4lb hammer to make belt splice on 12/4/16,during work activities EE states they strained wrist. EE worked w/o restriction until seeking med attn on 2/28/17 becoming a RLT on 3/1/17.Cause of the incident was lack of EE concentration. Employee was removing banding from a lift of roof screen, while removing banding the band sprang back resulting in a laceration to EE's arm causing employee to receive three stitches. Cause of the accident was lack of concentration. PPE was not a factor, mining equipment and mining systems was not a factor, compliance rules/regs was not a factor, miner proficiency was a factor.

While EE was bending over to pick up water hose they strained their lower back. EE worked 8/31/16 and became a RLT after seeking medical attention on 9/1/16. Lack of proper lifting technique was the cause of this incident. Employee was removing dollies from monorail on a previously worked out panel, while removing dolly from end of monorail, finger was caught between dolly and monorail. Employee was replacing a locomotive battery cell in a spare battery located on the ground, while carrying the cell employee overexerted EE's back causing a strain. Employee was hand spreading potassium permanganate onto pond for treatment when he slipped and fell on rocks getting some of this potassium permanganate into his eyes. EE seeked medical attention and received eye drops. Alleged incident-EE knocked #9cm breaker, was removing the cathead and received an electrical shock while their neck was against the cable. Incident is still under investigation. Cause is unknown at this time. While walking thru last open x-cut on at the longwall employee stepped in a hole twisting his right knee, employee began missing work on 12/17/15 as a result of this injury. Employee was handling belt stand during belt and power move when a bottom roller fell over striking his hand. Employee's lack of attention was the cause of this incident. Employee was assisting another employee while changing a vertical pin when a piece of metal broke free striking him in the wrist. Body placement was the cause of the incident. Employee was using a knife to cut a nylon winch rope when the knife slipped resulting in a laceration to the thumb. Employee's lack of awareness was the cause of this incident. Employee was checking sensor at a pond site when he slipped and fell, attempting to catch himself he injured his shoulder. Employee began missing work on 6/29/15 as a result of his injury. A roof fall occurred in 1 west bleeder at 84xc. Was approx. 8' H, 18' W and 25' L. Area was bolted with 8' point anchor resin assisted bolts, 12' CC cable bolts in pattern along with roof screen and bacon straps. Fall did not obstruct vent. or escapeways. Cause of fall was deterioration due to weathering. Floor/Roof support will be set as reported to prevent future occurrences.

Employee pulled golf cart into x-cut and came into contact with a bacon strap resulting in a laceration to his forehead. Cause of injury was not paying attention to surroundings. Injured employee was carrying 8" pipe with coworker when the coworker stopped and backed up causing the pipe to strike him in the face causing contusions.

Grating measuring 20"x42" had been removed from floor to perform scheduled maintenance,injured employee was walking back to area to perform work,he stepped into hole and struck his ribs against grating. Cause of injury was employee's lack of communication. PPE was not a factor,mining equipment/systems was not a factor,compliance rules/regs was a factor,miner profiency was a factor.

Employee was using a drill when the drill bit caught and spun the drill out of his hand striking the back of his right hand. Cause of incident was employee lacking to hold onto drill. A roof fall occurred at 79+25 in 4 west section in #2 entry inby the tail. The fall measured approx 32' in length, 16' wide and 15' in height. Area was bolted with install III resin assisted bolts, CC cable bolts in every other row along with screen and bacon straps. Contributing factor may have been physicals characteristics of strata and low cover.

A roof fall had occurred at 58 1/4 xc in 3 west in #4 entry. The fall measured approx. 10' in length, 17' wide and 5' to 6' in height. Area was bolted with 8' install III resin assisted bolts with screen, straps and 12' CC cable bolts installed in every other row on pattern. The return/alternate escapeway where re-routed and re-opened. Caused was deterioration of the mine roof.

Injured moved ram jack off of shearer went to pick it up, to get the ram out of the walk way. He went to place it on top of shield toe he smashed his middle finger between ram and double eye when he was setting it down. Injured was helping a co-worker move a 4" twenty foot piece of pipe and when his co-worker pushed on the pipe to walk, injured felt a popped in his left shoulder and pain. Injured was standing on the pontoon toes of long wall shield, pan was pushing his foot slipped off and got caught between relay bar and pontoon shields. PPE was not a factor, mining equipment/system was a factor, compliance rules/regs was not a factor, miner proficiency was a factor.

A roof fall had occurred on our #2 Main Belt at 8+83. The fall was 26 feet long, 17 feet wide and fell above the roof horizon of feet. Area was roof bolted with two piece roof bolts, headers and pie pans. Fall was caused by deterioration of the mine roof through weathering.

Injured was cleaning build up on belt structure with a wedge cap. The wedge got caught on top roller pulling right arm in between top roller and belt before he could pull belt pull cord to stop the belt. Employee was using a pick hammer to pound on his drill chuck to free up stuck drill steel on his roof bolter when a piece of metal broke off striking him in the forehead. Employee was changing vertical shearer pin, he laid a new pin on relay bar. When the ram pushed the relay bar snapped up in the air the pin laying on that relay bar struck employee in forehead cutting him. Injured failed to stop a track locomotive at a equipment door across mainline track. He hit the door with the locomotive causing the door frame to fall and lacerate his right elbow. Injured was trying to dislodge a roof bolt that was stuck under a long wall flight chain. It came loose and struck him in his upper lip. During a part 50 audit, it was determined this injury was reportable due to a fractured toe. Employee went to move a spool of cable at a corner, overhanging brow fell striking his foot. Injured was lifting a heavy plate with a helper felt several pops in his elbow joint causing loss of strength in lower arm. This injured became a lost time injury on 10/3/2013, he worked everyday until 10/3/2013.

Injured was carrying a piece of belt structure off the rib, turned to take it to the belt stepped in a hole fell striking the back of his head to the ground causing an abrasion to his head. Injured was hooking up flinger duster to scoop to take off hydraulics. He had position himself between scoop and flinger duster. He got himself between the scoop and fell on belt structure laying on the mine floor injuring his left knee. Injured was installing a car coupler on a mantrip, he reached down to get the pin out of the second coupler and the first one fell out of the slot on the mantrip smashing his middle finger. Injured was walking by a parked scoop car tripped on a loose track tie fell and hit his front tooth on the top of the scoop car causing his front tooth to be knocked out. Changing shield relay bar on long wall shield, shearer went by and pan line pushed out and the shield was not all the way off the top, the hog chain pulling it broke and hit him in the bottom of the foot. Employee was tied with a harness off of a monkey face welded to an I-Beam, the weld broke free allowing him to fall 8 feet to the ground. His head struck the ground cutting the back of his head. During a part 50 audit, it was determined this injury was reportable due to physical therapy. Employee was carrying curtain on shoulder, switch to other side, after picking up roll, he felt back pain. Cause was improper lifting. Injured was driving golf cart rubber tired vehicle on track rails, as he was coming outby a low hanging life line caught his face lacerating his chin. PPE was not a factor, mining equipment/systems was not a factor, compliance rules/regs was not a factor, miner proficiency was a factor.

During a part 50 audit, this injury was determined to be reportable due to splint applied. Employee was operating a rubber-tired vehicle, the tire caught a track switch and spun the wheel, knob striking his hand. Cause was traveling too fast for condition. Injured was hanging feeder cable in the belt entry, bent over to grab feeder cable a rock fell out between rib line and roof screen striking him in the lower neck and shoulder. Employee was assisting on the installation of track rail. One rail was jacked up and he had his hand under the rail to clean dirt from underneath of the rail. During part 50 audit this injury was determined reportable due to 2nd degree burn. Employee got burned when he went to removed flow switch on a booster water pump, approx. 2 cups of hot water contacted his skin. Cause of this injury was trapped energy (hot water). Cleaning up in the motor barn shop backed up at the work bench hit his arm against a bar of steel cut the back side of his right arm. PPE was not a factor, compliance rules/regs was not a factor, mining equipment/systems was not a factor, miner knowledge was a factor.

Spading a check curtain to the rib line and was using a roof bolt plate as a hammer to pound the spad in the rib. He missed the spad and struck his left hand with the roof bolt plate. Using track jack to raise critter went to lower the jack when the top part of the jack fell smashing finger between top of the jack and bottom part of the extension.
