Employee tripped over a water hose as he was attempting to close a water valve. The valve was a 4 inch gate valve with a locking handle. The handle had a sharp edge on it. Employee received a laceration to his right shin that required sutures.

He felt a burning sensation in his left knee, but continued working and did not report the accident untill the next day 01/10/2008. Employee was sent to local hospital on 01/10/2008,where he was diagnosed with a sprain to his left knee.

DURING A BLOW BAR CHANGE IN THE IMPACT CRUSHER, EMPLOYEE WAS STANDING NEXT TO THE ROTOR. THEY HAD CHANGED ONE SET OF BLOW BARS AND WERE GETTING READY TO MOVE THE ROTOR TO THE NEXT SET. CO-WORK ER MOVED THE WEDGE OUT. VICTIM SLIPPED AND FELL ONTO THE ROTOR. THE IMBALANCE OF THE ROTOR PULLED HIM DOWNWARD AND CRUSHED HIM.
