Employee states he was to guide 9016 shovel back into stall 17, he was in the stall facing west when he claims to have slipped on the right strip of rubber belting used to protect the floor. He did not report the incident until 12/31/09, he sought medical treatment on 12/31/09, we are unable to accommodate his restrictions.

Mostrando los 30Employee came down dust collector steps. Bottom step to ground level had uneven transition at ground level - it had been filled with crushed rock. Employee's boot slipped on bottom step to crushed rock area fracturing his right ankle.
EMPLOYEE WAS PULLING A BLOCK OUT FROM UNDER THE CRANE OUTRIGGERS AND SLIPPED ON LOOSE PELLETS. FELL TO THE GROUND AND FELT PAIN IN BACK AND GROIN.
Employee was removing plugs in preparation for loading blast holes. After he had removed one of the plugs, he turned on the "chip pile" to step down onto the roadway. As he stepped, his right ankle rolled. Employee reported that his bifocals contributed to poor depth perception as he stepped off the chip pile.
EMPLOYEE WAS DESCENDING STAIRWAY IN THE MAINTENANCE SHOP AREA. EMPLOYEE DESCENDED DOWN THE STAIRWAY UNTIL HE REACHED THE LAST STEP LOSING HIS FOOTING & SLIPPING OFF OF THE LAST STEP STRIKING THE FLOOR RESULTING WITH A BRUISED LEFT KNEE & SORE BACK ON HIS LEFT SIDE. EMPLOYEE ALSO STATED THAT HIS LEFT SIDE RIBS HURT. EMPLOYEE WAS SENT TO VRMC TO BE CHECKED OUT.
Employee was walking down the concrete ramp, employee had his hands on the handrails at the time and stepped on some loose pellets and slipped and caught himself. Employee alleged that he injured his Lower back at this time.
Employee sliped on ice, fell to knees twisting his lower back.
Employee was searching for hex mesh steel next to pile of rolled channel steel. He went to walk away when his pant leg snagged on the steel causing him to trip and fall on his right elbow.
Employee stepped out of the doorway at the roof entry point and onto a wooden walkway constructed of spaced 2x4s. Two of the boards were broken covered with OSB board. She stepped on the board, which apparently slipped and she twisted her ankle.
Pulling out screen on 2B4 primary screen to clean it. Using puller made to pull out screens, the screen tipped to one side causing him to fall back against 2A1 south finisher. The finisher had some rusty jagged metal which the employee fell against causing a laceration requiring 9 stitches. The employee is off work as of 4/17/08. Will file amended upon his return to work.
Employee took vehicle to Lundgren Motors for vehicle repair. As he exited the drivers door he slipped and fell. Doctor visit of 3/13/08 indicates hair line fracture with restrictions of 5 lbs lifting. Next appointment is 3/27/08. We will need to file an amended copy at a later date once restrictions have been removed.
Exited prod. truck at end of shift walked to coarse tails pocket. 20 ft from pocket slipped on ice fell on back. Feet came out from under him and he fell to ground. Employee will be off work for a few days depending on check with physician. He has wedging of th T6 vertebra or possible compression fracture.
twisted right knee on top step of access stairway of 9203 prod truck.Next step was onto the deck of the truck.he stepped up floor slipped twisting right knee.Sought treatment on 3/21/08 but told only to return to clinic if symptoms get worse.DR recommends surgery in June 2008. Will begin losing time when he has surgery
While pouring concrete, stepped backward, foot caught in wire mesh. Fell, twisted knee and jarred neck.
EE was putting the hose on vac truck and slipped and fell off wet back platform.
Slid down the slope in drying windbox, left foot went in the hole, twisted left knee.
Employee was going to the truck shop through door #8. He slipped on snow and ice at the door way and strained left knee. He continued to work since then with no restrictions no lost work days but has informed us that he is having surgery on 1/30/07 related to this incident. He begins losing time for this previous year's injury on 1/30/07.
Employee was changing a ripper tooth on 9231 D11. Used a 2-wheel cart, with the bottom lip of the cart used to pry up the bottom of the tooth to aid in alignment. Once aligned, a hammmer was used to drive the locking pin into place. When the pin set, the cart slid out, causing employee to lose balance & fall back, striking head against truck. Stitches were required.
Employee was performing PM on 9228 dozer. While loosening the filter with wrench he had to reposition himself to get better position to remove filter. His foot rolled back on track pad causing sharp pain in calf muscle. Employee was restricted in activity for one day before going on vacation for a week. Received physician notes on 9/22/05.
Employee was stepping onto fuel truck when on the first step up (apprx 20" off the ground) he felt something tear in the muscles of his right knee. He had just finished fueling the 29 loader.
Reported to work prior to start time. Went to unit 26 truck; placed lunch box on seat, then started to go back to operations office for job assignment. While walking felt he twisted his right leg on uneven ground in front of the electric shop. Felt pinch in calf and upper leg. Reported the problem later in the day.
Operating backhoe under line 1 cooler when he thought a track was jammed. He exited the machine slipped on pellets felt strain in back. He has injured the same body part as previously in April 2002 and prior to that - it appears it is an aggravation of several old injuries.
Employee was working on 5R rod screen. He was handing screen panels to another employee and foot slipped on a cover. He caught himself before he fell. He felt a sharp pain in lower back.

REFUELING 9763 D9 CAT. FUEL LEAK OCCURRED ON TRACK. EE NOTICED LEAK, STEPPED OFF TRACK WITH RIGHT FOOT ATTEMPTING TO AVOID LEAK BUT LEFT FOOT SLIPPED ON LEAK RESULTING IN FALL. FELL ONTO LEFT THIGH. DEVELOPED HEMATOMA RESULTING IN MEDICAL TREATMENT WEEKS LATER.
9526 PIT SERVICE TRUCK DIED. TOOK MAINT. TRUCK TO BONE YARD TO OBTAIN PART. TRIPPED & FELL INTO GONDOLA CAR WHILE LOOKING FOR PART. NO BED LIGHTS, POOR LIGHTING. NO TREATMENT/DIAGNOSIS UNTIL 4/8/04.
WALKING OVER PARKING BERM BACK TO FUEL STATION WHEN FOOT CAUGHT COMING DOWN BACK SIDE OF BERM & GAVE WAY UNDERNEATH HIS WEIGHT. FOOT STAYED PUT WHILE REST OF BODY KEPT GOING-FELT LEG SNAP
PERFORMING PLANT CHECK WALKING DOWN PUMP ROW. SLIPPED AND CAUGHT HIMSELF.
