Employee was holding rib mesh up with a 6' rod to keep self away from rib while mesh was bolted. EE went to step away and EE's foot was under a loose rock causing EE to fall.

Employee's right foot became stuck between the relay bar and the toe of shield 140. EE reached over their back towards the next shield and lost balance. Employee fell over fracturing EE's right leg.
An employee was attempting to crossover a pressurized water hose, when the hose burst. The employee then slipped/tripped over a 2" waterline on the ground. This resulted in a sprained/twisted ankle.
Employee arrived to work and entered the bathhouse through the boot wash area. He was ascending the steps. As he stepped onto the concrete floor his foot slipped and right knee buckled. As he fell to the ground he stated he felt his knee cap "pop". He was assisted into the office area for treatment.
Employee was hanging a block curtain by the feeder on the 3rd step of a 6 foot ladder. A service truck was backing up towards the feeder to drop off a spool of co cable and backed into the ladder knocking the ladder and employee to the ground. I.E. was on the cable tray working on the shearer. When I.E. was in the process of coming down his right foot slipped down into the cable trough causing I.E. to lose his balance and fall. This resulted in I.E. breaking the tibia bone in his right leg
Employee was walking by the stage loader at the longwall face and twisted his knee. At the time he thought it was nothing so no accident report was filled out. In May it started bothering him so he filed an accident report on 5/4. This injury became reportable on 6/4/15 when he had surgery on his left knee.
Employee was stepping between platform and mainframe on double boom roof bolter. Upon exiting platform his light cord got caught on his bolt tray causing him to stumble and fall. When he fell he tried to catch his self with right arm, feeling a pop in his shoulder
I.E. was on a ladder sealing a stopping. When he started to come down he missed a step. This resulted in I.E. falling backwards off the ladder on his back. This resulted in a back strain.
