Employee was loading shot when stood up right knee popped out of place. Employee reported this is not the first time his knee went out, happened in previous employment. MRI done on 7/10/12 showed weak Patella, employee was given a brace and set up for physical therapy. Since incident employee has been able to work within his underground miner 1 classification.

The plant operator was carrying two 5-gal buckets of lube oil to a cone crusher lube system. The operator shared that he was not paying full attention to his travelway when he stepped in a vehicle rut and twisted his right knee. The operator completed his shift, requested medical treatment the following day. An MRI and follow up reviled a sprained ACL.

HVAC contractor was applying caulk to the outer exhaust fan shield from a ladder. The ladder went out from under EE and his right index finger caught the sheet metal flashing around the fan shield. First aid was applied at the sign in room and his fellow employee drove EE to the Hospital where he received ten stitches and a referral to a hand physician.
