The Temp Contact Employee was walking down a dry dirt designated path when the employee misstepped and fell onto backside. The employee then got up, and felt a knot in right hamstring. Employee was later checked out and was diagnosed with a possible muscle strain and returned to work the following day.

Mostrando los 4EE were re-positioning the Trommel to complete a repair. As the Trommel moved, the feed chute for the trommel swung down and struck the ee on the arm knocking the ee to the ground. This fractured the ee arm is several places as well as lacerating it. First aid was administered on site, emergency services were called and ee was taken to the hospital.
Upon getting on the side stairway of service truck #342, while attempting to maintain 3 point contact by turning around at top of stairs to exit truck as per MSHA recommendations, EE lost his balance while holding the right side hand hold and impacted the top of side box with left elbow causing severe pain to left joint area.
While drilling a hole in the nuggett trap chute to secure riffle, employee slipped from 2 step step stool causing him to fall landing on the platflrm the step stool was on. Landing on his back.
