Mostrando los 5The injured employee was installing a fill valve assembly on a product tank. As the employee was adjusting the valve with a 24" pipe wrench it slipped off the valve assembly causing employee to fall forward into the side of the tank. As employee fell forward felt a pop in right shoulder.
