Employee was cleaning material buildup on the grizzly screen of a feed hopper. When attempting to step off of the grizzly bars EE's right leg fell through the grizzly bar openings (about 6" opening), causing injury to EE's right knee after hitting the metal bars.

Mostrando los 10Employee was walking down a flight of stairs from the Conditioning Tank down to Cleaner #1 in the mill when they slipped/tripped on a step causing injury to their left leg (knee, shin, and foot).
A process operator was pushing material with a dozer at the tailings stack when they exited their equipment to clear off accumulated rocks/materials from the track undercarriage. The employee turned to re-enter the dozer and tripped over a rock. The employee extended their left hand during the fall, taking the full weight of the impact onto their hand wrist.
Employee was removing a press cloth from press 2 on the second floor of the press house that was full of mud. The cloth released during movement of the cloth and the force being used was suddenly no longer in the range of motion and the employee pulled EE's shoulder.
Employee was walking to work area lost EE's footing and slipped on loose gravel. EE's immediate reaction was to reach and grab for a wire running along the Rib. In the process, EE dislocated EE's right shoulder.
Employee stepped foward with left foot, pushing off with right foot. Employee heard a loud pop. The 4th and 5th toe of right foot are broken. The ground the employee was on was uneven.
EE sprains knee when EE felt knee pop while dismounting water truck. EE transferred weight from one foot to the other on factory truck steps and lost balance, falling to the ground.
During bolting activities the injured miner was attempting to get out of the way of a rock and was either struck by the drill of another miner or fell into bolting supplies that were leaning against the shaft wall. The miner suffered a lacerated nose and scraped elbow.
Employee reported twisted ankle while dismounting a Skid steer. No light duty, no fracture, no splint. MSHA insists this is reportable.
Descending stairs. Stairs were clean and clear and he misstepped, causing sprain to right knee area.
