Employee was cleaning cat walk around Jaw area
The injured employee was at the washing plant to clear debris from the catwalk and screening surface. While walking on the catwalk employee slipped on ice and fell.
It was the start of the shift and employee was going up the steps to enter the control room for the start of the shift. As they reached the top step they turned to talk with a coworker. As they turned they slipped backwards and Right foot became caught between the stairs. This prevented them from falling down the stairs, but it injured the foot.
Our maintenance mechanic was doing pre-operational inspection and preventative maintenance on a Hitachi EH 1100-5 haul truck on the morning of Wednesday, April 19, 2017.
