Miner was standing on a box attempting to locate a part. As EE stepped down the box caved causing EE to fall back. EE's head struck the floor. EE's initial report of injuries was pain in neck and lower back, along with a laceration to the back of EE's head. EE was transported to Atrium Health for scans and X-rays. Miner was cleared with 4 staples and released for work.

EE stepped off a concrete pad (6") and rolled right ankle on 5/13/2025. EE has worked with discomfort in it since then. EE wanted to be seen for the lingering pain on 6/24/2025 and was released to work with weight baring restrictions making this accident MSHA reportable.

IE and co-worker went up to the 3/4 stacker feed belt to get material off due to it being overloaded. After they were done IE went to step off onto the ground and when IE did IE's foot caught the side of a rut and their ankle twisted due to the ground being frozen.

EE was climbing on excavator after changing hydraulic hose to retrieve rags. While climbing down EE missed the last step with EE's foot and fell to the ground. EE landed on EE's right side, EE's hard hat came off and EE hit the back of EE's head on a rock. The wound required 4 staples.

Traveling a path without clearing it first, and doing so with hands full of materials.

Employee said was climbing onto the 824 J John Deere Front-end articulated loader. EE's foot slipped off from the first step approximately 24 inches. EE was unable to hold onto the grab irons, falling backward onto back. EE also said had muddy boots.

The EE was going to the pit to check on a haul truck driver that said truck was overheating. While in route, the superintendent told the EE's NOT to take the radiator cap off. One of the EE's did, burning the injured EE's left elbow and forearm. The injured EE jumped off of the truck and fractured EE's left heel.

Employee climbed steps to the 1/2 and 3/8 belt platform to get a rock sample. When EE stepped onto the platform, EE rolled EE's right ankle and had to catch self.

Employee was walking down the stairs. When EE stepped to ground level EE twisted EE's left ankle.

Employee was attempting to mount a aerial man-lift. As EE was stepping up holding on to the handrail, the other leg slipped on rocks out from under EE. Holding on to the handrail the sudden jerk to EE's arm, strained right shoulder.

Employee was walking out from the breezeway hall, where the weight room is located underneath the 3 and 4 Kilns. As EE stepped down to a lower concrete level, EE twisted left knee. No debris in the area.

Employee heard 3/4 silo belt lock down, ee went to the control room to turn it off. EE then went to investigate what was wrong, ee found some rock had wedged and locked belt down. EE went to get a ball pin hammer. As ee was returning to the conveyor, ee stepped onto uneven ground, ee lost balance and fell against the silo.

Employee was walking carrying two buckets of castable material, to be used to patch holes inside the #1 rotary kiln. Employee stepped onto uneven ground twisting the right ankle.

Employee exited the shipping department control room using the stairs. At the bottom was some more concrete stairs, when EE stepped down EE rolled EE's ankle.

Employee was walking around the rear of the articulated front end loader, inspecting due to the loader over heating. EE slipped on the snow/ice strain EE's left knee.

The injured employee was walking to turn off the augers power switch. Employee twisted employee's ankle when employee stepped over a 6 inch steel pipe.

Employee was installing a grease O-ring seal to a rock crusher. The O-ring slipped off from the back side where employee started. Employee fell forward to the edge of the brass socket liner.
