While stepping out of pick-up employee slipped on small frozen puddle. Employee fell congtacting pick-up causing fracture to left forearm. Employee was given light duty restrictions and returned to work on next scheduled shift.
while mounting a lift truck employees foot slipped off the step resulting in EE contacting the truck with EE's right side lower rib area was seen at ER and given full release. On 7/5/24 EE went back in for soreness and x-rays were negative but restrictions were issued making this reportable on 7/5/24
Employee had just completed washing down the work station floor and as EE was rolling up the wash hose EE slipped on the wet floor resulting in left knee to pop followed by pain. Sent to the ER then onto an occupational health physician where EE was issued restrictions for left knee strain.
Employee slipped after showering and employee's head contacted locker door resulting in a laceration. Was seen at ER where sutures were required. Full work release was given making this a medical reportable incident.
Employee doing routine maintenance work at the past fill plant on the mixer paddles when EE's footing slipped resulting in the employee falling against mixer paddle resulting in a closed fracture to one of EE's right ribs
Employee was in the process of replacing a portable pump in EE's work area & as EE lifted the new pump from vehicle and turn to carry it to install location EE rolled EE's left ankle. After installing the pump pain continued to increase, informed supervisor, seen by site medic, referred to ER, x-rays positive for a fracture.
Employee reports stepping of CMAC and rolling ankle. Employee did not immediately report in an attempt to walk it off. Employee reported it at the end of shift and was seen by site medic, referred to ER and then a follow up with Occ Health on 3/27/24 and was issued restrictions on that day.
Employee pulled a drisco pipe into the heading & after they had unchained it from the loader they stated as they stepped backwards with left foot it didn't move as if it was stuck causing them to fall. Employee called out to their partner as they had pain. Seen Site medic referred to ER, x-rays were positive for left ankle fracture.
The employee reports slipped and reached to catch self on the mucker bucket. In the process EE's fingers were caught between bucket and roll back cylinder of a 2 yard mucker pinching two fingers.
Employee was mapping drill hole break throughs & in the process EE was stepping/walking backwards when stumbled resulting in EE falling backwards, EE put hands outwards to break the fall resulting in a sprain to right wrist. Seen site medic referred to ER x-rays were negative, but restrictions were issued for acute sprain.
Laborer was walking on screen at MSE wall and caught EE's left boot on the metal screen and tripped and fell. The employee hit EE's left knee on the ground causing a laceration.
Employee was walking to open a rail gate when EE tripped and in the process of regaining balance EE felt pain in left knee. Continued to work until pain levels increased. Seen Site medic, referred to ER, x-rays were negative, referred to orthopedic where restrictions were issued making this incident reportable
Employee states: was removing cable hanging on the rib approximately 6' above the sill. With hands over head, lifting the cable EE's foot slipped causing a "pop" in right shoulder. The employee required an MRI and a follow up appointment with an ortho dr. on 10/3/23 and was diagnosed with torn shoulder ligaments. EE was taken off work to rehab the shoulder.
Employee was exiting a covered mantrip on the surface when EE lost EE's footing & fell resulting in a dislocation & laceration to right middle finger. Seen site medic, referred to ER, X-rays negative. Sutures were required and was referred to a orthopedic surgeon for dislocation, no restrictions were issued with surgeon making this a medical reportable incident.
Employee was getting off the company provided bus in the parking lot, upon stepping down on the asphalt employee's left ankle rolled, resulting in a sprain prior to starting shift. Seen site medics, referred to ER, x-rays were negative but restrictions were issued for the swelling.
An employee had arrived at the mine site parking lot and upon exiting EE's vehicle EE slipped and fell, resulting in injury to EE's left ankle. Seen site medic referred to ER, x-rays were positive for a fracture, restrictions issued.
Employee at a raise bore drill site went to pick up a cordless tool battery when EE tripped resulting in EE putting EE's hands out to catch self resulting in an injury to EE's right thumb. Seen site medic, referred to ER, x-rays were positive for a fracture resulting in restrictions.
At the start of the employees shift EE was walking from the Administration building to the Hoist House & slipped on the snow resulting in EE falling onto EE's left knee resulting in pain. Seen site medic referred to ER, X-rays negative, referred to Orthopedic Specialist where EE was given restrictions at this time for strain to knee.
Employee was using the empty prill pot hose to blow out drill holes prepping for loading the face. In the process employee tripped over the hose while cleaning hole causing EE to fall into the face resulting in lacerations to EE's right elbow and EE's scalp after hard hat fell off, sutures were required, full work release issued making this a medical reportable injury.
Employee used the rope provided on the rail switch arm to throw the switch to a dump. After throwing the switch, EE started to walk back to the loci & in doing so EE's foot contacted the rope causing EE to trip & fall resulting in injury to EE's left wrist. Seen site medic, referred to ER, X-rays positive for fracture, restrictions issued.
Injured employee was dismounting Jumbo JD059 from the operators cab to check a setting at the back of machine. During the dismount process the employees heel slipped on the provided step and hit the ground with EE's heel, rolling EE's ankle and proceeded to fall to the ground.
While sitting on the bench to remove EE's sock the employee's foot slipped out from underneath EE. The motion caused EE to slip off the bench. EE caught self with EE's left arm on the bench causing injury to EE's shoulder. Evaluated at PMC and issued a full release with a shoulder strain. Follow up on 3/16/22 w/ Ortho. Diagnosed with torn shoulder muscles. Issued restrictions.
Employee was checking supply flats on surface for specific parts. While walking near supply flats employee slipped on the ice and struck head on the ground. The impact caused a laceration requiring sutures to close. The employee was issued a full release and returned the following shift.
Employee was conducting post blast inspection, wetting down a muck pile. Employee tripped and fell injuring left shoulder. EE was seen by site medic, referred to ER and then to Occ Health where EE was given restrictions.
Employee had exited EE's vehicle and was preparing EE's vehicle bed for a load of shot-crete when EE rolled EE's right ankle when pain persisted EE reported to supervisor. Seen site medic, referred to ER, x-rays were negative but restrictions were issued making this reportable.
Employee was packing a box of powder to the face when EE stepped on a small rock, rolling their ankle and falling. Used right arm to brace for the fall, EE felt a pop in the right bicep. Finished loading & reported at end of shift, Seen at ER, Positive for right bicep tear, restrictions issued.
While pulling the jackleg out of a low bolt hole on the rib, the employee stumbled and fell. The jackleg also fell contacting the employee on EE's ankle. The result of the incident was a chip/fracture to the distal end of the tibia.
Employee was exiting a haul truck when foot slipped off the step causing employee's head to contact the door frame jarring employee's head & neck. Was seen at ER and given full release, at start of shift on 4/9/21 employee was still having neck pain and was seen a 2nd time by doctor and was given restrictions at this time.
Employee was walking from EE's locker to the shower facilities when EE fell causing a strain to EE's left thigh muscle. Seen at ER and restrictions were issued.
Employee was descending a set of stairs. EE was carrying a bucket in one hand and holding the hand rail with the other when EE's left foot slipped on the step causing EE to fall into a sitting position with the right foot under EE. EE was seen by site medic and referred to the ED. EE was diagnosed with a fracture to the rt fibula. Referred to Ortho for follow up.
Employee was walking on sidewalk From Admin. building to parking lot when EE slipped & fell. EE got up and went home. Later that evening EE informed EE's supervisor & was instructed to be seen by a physician. X-rays came back positive for a right fractured rib EE was given a full work release & making this a medical reportable injury
Employee was standing on the back of a flatbed trailer used to haul vent-tube when EE's partner pulled the tongue pin causing the trailer to move causing the employee to fall approximately 2 feet onto EE's right upper back and shoulder area. Seen by site medic referred to ER, restrictions were issued for contusions
Employee tripped & in catching self so as not to fall EE reached out & grabbed wire mesh on the rib. In doing this EE hyper extended left thumb, seen site medic & released, next shift it was swollen & could not bend, seen at ER, referred to Specialist, X-rays revealed hairline fracture, restrictions were issued.
Employee was climbing a ladder while performing water line maintenance in the 65 722 shop. EE mis-stepped on the ladder causing EE to contact EE's chin on the ladder. EE was seen by the site medic, referred to ED for sutures to close the laceration and was issued a full release to return to work.
Employee was wetting down a muckpile & as EE went to back up EE tripped causing EE to fall. When EE landed, left elbow contacted a rock causing a laceration. Seen site medic referred to ER, sutures were required, a full work release was issued making this a medical reportable.
EE was completing the installation of an auto header system for the alimak. EE had climbed up a ladder and then stepped over onto a hose reel to wire tie the hoses to the back. EE felt the hose reel start to move so EE decided to jump from the top of the hose reel, landing with EE's right foot sliding under the hose guide, fracturing and spraining EE's right ankle.
On 5-10-20 an Employee stepping out of passenger side of vehicle when EE slipped twisting right knee. Seen by ER & referred to next level of care. Due to Covid 19 protocols not able to be seen until 5-19-20, a MRI was ordered & scheduled on 5-22-20, results were available 6-10-20 where operator was informed of a positive tear to meniscus tendon making this reportable
Employee had exited the mine administration building after finishing shift and as walking on the sidewalk to the parking lot employee stated they slipped on a slick spot causing EE a slip and fall injuring lower left leg. Seen by site medic, referred to ER, x-rays were positive for a closed fracture, restrictions were issued.
Employee was operating a jack leg and the leg was extended. Lost balance and fell. EE's hand was still holding on to jackleg handle and was caught between the handle and a rebar bolt that was stood on the rib. Employee was seen at ED. X-ray negative but did require 3 sutures to close laceration.
Employee was dismounting Toyota Man Carrier when EE believes stepped on a loose rock causing leg to fall out from under EE. EE fell to the ground on knees. EE was referred to ortho and was diagnosed with a torn meniscus in the left knee.
Walking down excavator platform steps, mis-stepped, "rolled" right ankle. This accident occurred on the last step of the platform. EE was seen by EB Medics and referred to the ED where EE was diagnosed with a fractured RT ankle/foot.
Employee was stepping off a Young Buggy from the operator's cab & felt a pop in right ankle. Seen site medic, referred to ER, X-ray's were negative, & a full work release was issued. Injury was still bothering employee, went to a physician at which time restrictions were issued for right ankle sprain on 11/5/2019 making injury reportable at this time.
While changing out bucket on CX 130C excavator employee slipped on snow in bucket. Causing EE to fall and contact ground with right elbow. Seen by sight medic referred to the ER. X ray positive for chipped right elbow.
While walking back to transportation from a sump the employee fell landing on right buttock and strained groin in the same leg. The employee was seen by the site medic and taken to a Medical Center where employee was issued restrictions.
Employee reported on June 7th that on June 6th when employee stepped off a diamond drill platform and that employee rolled right ankle. Seen site medic and referred to ER where restrictions were issued
The employee was reporting to work on night shift. As EE reach the top step to the office and dry area EE stumbled, catching EE's fall with EE's right hand. In so doing EE dislocated EE's rt. small (5th) finger on the grating.
At the 23w room at laydown, employee was unloading supplies on concrete pad when EE took a step backwards causing EE's left knee to buckle. EE was seen by site medic referred to the ER and further referred to orthopedic where EE was issued restrictions pending the MRI results. EE first missed shift was 4/1/19.
While performing a workplace inspection, employee stumbled while looking the back over. EE landed on left side and received a laceration to left buttock. EE was seen by site medic and referred to Pioneer Medical Center. EE received three sutures and a full release.
Employee was exiting a man trip and in the process of stepping down to the ground twisted right ankle. Was seen by site medic and referred to ER. X-rays were negative but restrictions were issued.
