On 4/1/23 Miner slipped and fell on ice, diagnosed as PCL sprain will heal on its own. Miner reevaluated as knee was not improving and had an outpatient procedure performed to repair PCL on 8/31/23 and allowed to return to work but prescribed to go to physical therapy. No days were missed from this injury.
On 2/9/23 Electrical Planner slipped on ice walking around substation repair. No treatment. On 2/24/23 not improving went to doctor and diagnosed as sprain. No improvement and MRI taken with no findings. Another MRI taken 3/8, on 3/16 diagnosed as a muscle tear. Outpatient procedure performed on 3/16 applied brace, released to work and prescribed physical therapy.
Operator was performing a walkaround inspection on a blade and slipped on the snow-covered surface and fell backwards resulting in a knee injury. The Doctor has removed the employee from work and prescribed a brace to immobilize the knee. An MRI has been scheduled for 4/23/21. The first scheduled shift missed will be 4/23/21.
Employee was walking from the main shop to the truck shop. While walking employee tripped on uneven ground resulting in twisting an ankle. ER visit employee received a work as tolerated release with a stabilizing boot to protect a tendon in the ankle. Physical Therapy will be scheduled, employee is unable to perform job task, the first scheduled shift missed is on 02/27/21
On 02/21/21 employee was waiting for a crew van, slipped on the ice and fell causing back pain. On 2/25/21 employee notified the mine of MRI results, which showed small bulge in disc at L5 and S1. Physical Therapy ordered for 03/10/21 and taken off work. The first scheduled shift missed was on 02/26/2021
Guard shack attendant stood on a chair to reach an upper shelf, fell landing on left shoulder. Was diagnosed with a re-aggravation of a previous injury to the left rotator. The employee was going to be given a work as tolerated release but requested to be removed from work activities until released post operation on 9/10/2020. Surgery will follow when it can be scheduled
