Employee stated that while attempting to drain oil out of the 34103-rock truck, EE stepped into deep mud and fouled EE's foot. When EE attempted to pull free from the mud, EE heard EE's knee pop. Employee was treated by an orthopedic surgeon and taken off work pending surgery. Medical data was made available to the company on 3/31/26 that Part 50 requires to report.

Mostrando los 8Employee was walking down a slight grade when EE slipped and fell, injuring EE's right shoulder. EE returned to work the next scheduled shift, under restrictions for 2 weeks. This is reportable due to EE's job change for the 2 weeks of restrictions.
Employee was filling up grease bucket on lube truck. EE slipped and fell on spilled grease. EE's hand landed on a track shovel cutting EE's hand. employee received stitches in EE's hand.
While descending ladder on loader, employee misplaced their foot onto the outside bracket of a step; leading to rolling of the ankle and then falling to ground.
On 8/26/25 employee was climbing down a 8-foot ladder and thought they were at ground level and stepped backwards from 2nd from bottom step. This caused Tech to fall to ground and felt pain in their shoulder. Discomfort did not go away, and an MRI was done on 9/18 and results on 9/19 of a hyperextended shoulder and prescribed physical therapy to relax muscles and tendon.
Contract employee was filling a 5-gallon bucket of DEF at fuel island for a dozer in shop. On the last of three trips, walking back to vehicle (5-6 ft. away) with bucket, contract employee slipped and fell on snow packed and covered ice. Strong winds were blowing snow clouding visibility to walking path.
On 8/21/2023 Employee was performing a pre-shift inspection of a motorgrader. After inspecting the circle, the employee stepped back onto uneven ground and fell. Employee braced for the fall and stuck out their right arm. This resulted in pain in the right wrist, an x-ray determined a fracture in the Scaphoid. Doctor stated fracture will heal with no treatment and returned to work.
Employee had filled tank on truck and was taking fuel hose out of tank and as EE stepped down from truck EE slipped and twisted left ankle.
