Mostrando los 14Due to cooler weather, plant having problems with belts, pulleys and material freezing up. Conveyor belt started tracking hard to south side. EE was looking at tail pulley for material build up and slipped, twisting EE's ankle.
Twisted and sprained right ankle while walking slope
Electrician was installing a pump in a sump located outside on the hill ramp leading to the pit. He slipped into the sump while installing the pump, caught himself on the cable to the pump, tearing muscles in chest area. Did not report accident till 9-2-14, after visits to hospital
EE was checking the sight pole for an excavator operator. Some of the stone below his feet gave way and he fell into the ditch. He continued to work the rest of the day saying he was fine. He decided to go to the doctor on 4/22/2011. EE is part-time and any time off was not related to this injury.
Two workers were putting together 6" diameter yelomine pip. EE got his foot stuck in a strap, which caused him to trip, twist and fall on the ground. The resulting injury was a broken fibula in ee right leg.
Operater of a 980F Cat loader was checking a possible problem with his loader. He slipped and fell off of loader, breaking his left ankle.
Our company mechanic was doing some repairs on our Mechanical Scaler when he turned to dismount machine and lost his footing, falling 4 feet to the mine floor. The accident resulted in an emergency room visit where he received 9 stitches in his right ring finger and an xray to arm and shoulder, then released to restricted duty for 10 days.
Employee was attempting to wash out the bucket on the 980 Wheel Loader he was operating and while pulling on the 1 1/2" wash out hose, lost his footing and twisted his ankle. He was taken to the hospital for treatment. Upon inspection of the area, the ground apppears relatively flat. The weather was sunny and dry. The ground may have been wet from spraying the bucket out.
Stepped on loose ladder on left rear of the machine and fell to the ground.
The truck driver slipped and fell into a ditch that was being excavated from which he was hauling away the materials being removed to make the ditch, dislocating his left shoulder.
Scoop operator had pulled conveyor tail piece outside the underground mine took up for storage stood on top of it to cross it, stepped off of it and broke.
Lifting the ROPS from the piece of equipment. It sprang free, striking the mechanic and causing him to fall (93") to the ground.
WHILE LOADING BELT STRUCTURE INTO SCOOP OUTSIDE OF UNDERGROUND MINE, EE TRIPPED OVER THE EDGE OF THE SCOOP BUCKET AND FELL. HE INJURED HIS LEFT ELBOW WHEN HE TRIED CATCHING HIMSELF, STIFF ARME D AGAINST THE GROUND. CAUSE OF INJURY WAS AWARENESS. COMPLIANCE WITH RULES & REGS. NOT A FACTOR. PROTECTIVE ITEMS OR TRAINING NOT A FACTOR.
