Employee was climbing off the dozer that was located on the deck of the lowboy trailer while stepping backwards when they fell off the trailer approximately 3 to 4 feet to the ground onto their back. This resulted in pain to the right elbow, and the wind was knocked out of them. The employee was transported to a higher level of care for further evaluation.

Employee was carrying a receiver plate and tripped on a mixed gas hose and fell with the plate and the plate contacted the employee on the left eye brow. This caused a laceration over the left eye brow needing stitches.

EE was working on Tub of a dragline. He was standing on the 3rd step of a 6' ladder. As he went to step down off the ladder, his foot slipped on the 2nd to bottom rung. When his foot slipped, he struck his chest on the Tub Manway Portal in The Tub.
Employee slipped off of the lower step on a fuel truck and landed on the door threshold, injuring his right back rib area.
While getting down from a piece of equipment, employee stepped onto the ground and released his hands from the machine. As his weight transferred to his foot, employee twisted his ankle. Further medical evaluation, at a later date, determined this to be a restricted duty case.
