Employee was climbing off the dozer that was located on the deck of the lowboy trailer while stepping backwards when they fell off the trailer approximately 3 to 4 feet to the ground onto their back. This resulted in pain to the right elbow, and the wind was knocked out of them. The employee was transported to a higher level of care for further evaluation.

An employee stopped for lunch break at the 2 high wall rock run. Employee climbed down the ladder grabbed both chock blocks, was walking to set one chock block on the left front wheel. The employee slipped on the ice and landed on their arm.

Employee was carrying a receiver plate and tripped on a mixed gas hose and fell with the plate and the plate contacted the employee on the left eye brow. This caused a laceration over the left eye brow needing stitches.

Employee was removing the tin off of a portable building. When EE went to move EE's lanyard to a different place the tin EE was standing on gave way. The employee fell approximately 12.5ft.

Employee was walking down the belt line when EE stepped into a water hole and twisted EE's knee

Employee was climbing up the off side boarding ladder on 222 track hoe when, they slipped hitting right shin on a rung of the ladder causing a laceration on shin which required sutures.

While fueling 4117 pump in ramp 60, employee went to grab EE's radio out of 1771 lube truck. While climbing up steps employee slipped and hit EE's left side on steps.

Contractor employee was loading belt structure onto a trailer. As ee stepped off of the back of the trailer onto some uneven floor, ee rolled ankle. Contractor employee is placed on restricted duty.

Contract employee was pulling feeder cable slack to the feeder breaker. As employee was pulling employee fell towards the rib and land on employee's SCSR unit. The fall resulted in a fractured rib.

Employee was crossing pan line and employee's foot slipped causing an injury to employee's penis when groin area hit pan line.

Employee dropped a beltline down to splice the belt. Employee pushed the belt slack with left hand, while pushing employee stepped backwards onto a 4" lip in the mine floor. Employee lost balance and fell onto the left hand, this resulted in a dislocated finger.

Employee was walking up a rampway at overcast location. Employee had left hand on hand rail and carrying shovel with right arm. As employee was walking, slipped and fell on shoulder causing dislocation of shoulder. Employee has been placed on restricted duty.

Employee was walking in the reclaim tunnel, as he was walking he slipped on some ice and sprained his ankle.

Employee was pulling a piece of ventilation brattice down an entry, as he was pulling the brattice, he slipped and fell. As he fell he put his arm out to catch himself, he felt a pop in his shoulder.

Employee was installing panels for an overcast and in the process a panel collapsed allowing the employee to fall. EE has been placed on restricted duty.

Employee was moving cones for a scheduled blast, he slipped and fell on some loose material at ramp 60. The employee used his right arm to break his fall and heard a pop in his right wrist area. Resulting in a sprain to his right wrist and elbow.

Employee was loading dust pipe onto a trailer from the beltline. As employee was loading the pipe, he slipped on the wet surface and twisted his back.

Employee was climbing on to a D10 bulldozer to unplug a heater extension cord. As he was reaching for the extension cord plug, he slipped and fell to the ground. Employee was unable to return to work after medical evaluation.

Employee was carrying two bags of rock dust on his shoulder. As he was carrying the dust, his foot became caught in some mud resulting in pain in his back. Late reporting due to further medical evaluation which placed him on restricted duty.

Employee slipped on ice as he fell to the ground he fractured his left Fibula.

Employee slipped on ice and fell to the ground. As the employee fell backwards he turned to his right, landing on his right hip resulting in a contusion and injuring his Sacroiliac Joint.

Slipped on ice beside shop building and tried to catch himself with his right hand, straining his right shoulder.

Stepping from upper brace on structure to lower brace and foot slipped twisting ankle.

Employee was roof bolting and just finished drilling a hole. As he was inserting a tube of resin his foot slipped and he fell. His back hit the foot of the ATRS. As he tired to stand up he lost consciousness for a few seconds.

Employee was pulling a conveyor roller set off of beltline structure. As he was pulling, he tripped and twisted his knee. Late reporting due to further medical evaluation, which has placed the employee on days away from work.

Contractor was engaged on changing out 90 degree elbow on ventilation tubing, a "T" would replace the 90 degree elbow. He was using the removed elbow to stand on while installing the "T". He stepped off of the 90 degree elbow and twisted his ankle.

Employee was opening a mandoor located within the airlock equipment doors. The outer set of doors were open. As he opened the door he fell on his knee caused by the air pressure. He later sought medical attention and has been placed on restricted duty. Late reporting due to the medical attention that he later sought.

EE slipped and fell injuring his right shoulder, while pulling a barge pump to sure for repair at ramp 60 pond. His right rotator cuff was torn during the fall.

Employee was climbing off a D 11 T Caterpillar dozer and slipped off the right side draft arm step causing him to twist his right knee.

Employee was performing fireboss duties, as he was walking past a belt drive location he slipped and felt pain in his knee. He has been placed on restricted duty.

Employee was walking away from a shop car, as he was walking he slipped causing his knee to twisted. He has been placed on restricted duty.

Employee was walking to the back of company #749 water truck and slipped on some ice. As she began to fall she grabbed a hose on the water truck with her left hand causing her to strain her left shoulder.

Employee tripped while stepping over dragline power cable and fell on his left hand and left ribs. The employee was seen by the on shift ERT. The employee did not go to the hospital. On 01/10/2014 the employee seen his doctor and it was determined that the employee received an Acute, non-displaced, fracture of his left 8th 9th 10th ribs.

Employee fell backwards from an elevated platform and hit his head on the ground causing a concussion and laceration to his head.

Contractor was helping in the installation of water line. He was asked to get components for the water line, contractor turned around and slipped and then felt pain in his back.

Employee was walking in the 11th Right Section, while walking in muddy conditions, he lifted his foot and felt pain in his back and leg. Further medical examination on 7/24/2013 placed the employee on restricted duty resulting in late reporting of this injury.

2 employees were dragging a pump, controller and power cable along the offside of the belt, Employee tripped and tried to catch himself. Within doing so he hurt his lower back.

After sliding a steel I-Beam, employee stood up and a piece of rock struck him in the head, this knocked him down and he struck his face on the steel beam. He received stitches to close the wound and a broken tooth.

EE was hanging tube when the end fell out sucking him toward the fan, and hitting his chin. EE received a laceration to the chin and required stitches. EE was wearing his hard hat, safety boots, ear protection, safety glasses, and gloves at the time of accident.

EE was setting up to grade structure on the main west belt. EE was hanging a come along while standing on a ladder when he slipped and fell off. He landed face first, and injured his lower back. The ladder was set on level ground floor to top approx. 9'. Factor contributing to incident was inattention and improper method.

Two employees were lifting a piece of 1" plate steel, they were carrying it from the ground into the back of a pick up truck. One employee slipped in the snow, this allowed the plate to fall onto the injured miners foot. The injured miners foot was bruised from inby the steel toe portion of the boot. He was placed on restricted duty for two days.

Packing roller MW#21 tripped with roller on shoulder, fell, roller struck right hand cutting pinky finger on joint that connects to hand.

Employee was walking across the shop when he slipped on the track rails. As he caught himself he felt pain in his lower back.

EE reports he was shoveling on the tailgate of the long wall when they heard some rock falling and took off running and tripped over one of the shoes on the longwall system falling and straining his neck and back.

While walking down the longwall face, employee tripped and hit his finger on a piece of coal, fracturing his little finger. Late reporting due employee not seeking medical attention until 06/30/2011

EE was working on Tub of a dragline. He was standing on the 3rd step of a 6' ladder. As he went to step down off the ladder, his foot slipped on the 2nd to bottom rung. When his foot slipped, he struck his chest on the Tub Manway Portal in The Tub.

Employee slipped off of the lower step on a fuel truck and landed on the door threshold, injuring his right back rib area.

While getting down from a piece of equipment, employee stepped onto the ground and released his hands from the machine. As his weight transferred to his foot, employee twisted his ankle. Further medical evaluation, at a later date, determined this to be a restricted duty case.

While taking a shower the employee slipped and hit his head on the floor. This injury required stitches. He was released for regular duty.

Employee was turning on a water valve, stood up and fell over onto beltline structure, hurting his ribs. Original medical examine and x-rays showed no rib breaks, later medical evaluation showed rib breaks. Employee is on restricted duty starting February 17, 2009.
