EE came in on 4/19/12 said he had been to Dr. and that the Dr. wanted to do surgery on back as the result of a fall on 3/1/12.EE missed work on 3/12 & 3/26 but did not report it due to an injury. Employee had been operating 992G cat end loader and when he stared down the steps his left foot slipped and he fell and his left knee folded up under him - He fell with knee twisted up under him.
EE was using a chain saw to cut a tree approx 18-20" in diameter. When the tree fell, a grapevine that was unseen in the top of the tree pulled a dead tree approx 6-8" in diameter down and struck EE, knocking him to the ground.
