When employee released the brake on the rail mantrip, the mantrip began to roll forward. Employee panicked and jumped off. EE slipped and struck EE's left shoulder on rib/brow.

Employee was tramming a forklift when EE drove off the road into the ditch. The forklift leaned on its side against the berm. As the employee was getting out of the forklift EE slipped and rolled over the embankment approximately 15 feet.
Employee had just returned to the surface and while standing in the parking lot, employee felt a sharp pain in employee's right knee, employee stated - employee's leg "gave out", causing employee to fall to the ground.
