EE was attempting to reposition a metal section of the vent tubing so that the tube mated better with the section already mounted in the back. EE was wearing belt & lanyard but when attempted to reach a clamp he loosen his lanyard & he stepped on the rail to reach the clamp; when he reached the clamp & cable, the tubing gave away & fell. The EE fell on vent tubing and then ground.

EMPLOYEE SLIPPED ON ICY EQUIPMENT AND CUT RIGHT ELBOW REQUIRING THREE STITCHES.
EMPLOYEE FELL O0F BELT ONTO SCREEN BELOW.
I.NAME WAS PARK3NG HIS LOADER, AS HE WAS COMING DOWN THE LADDER HE SLIPPED AND HELD ONTO HANDRAIL. HE THEN SWUNG INTO THE TANK AND BROKE HIS IBS RIBS, HE DIDNT REPORT THIS UNTIL MONDAY 12-09-8 5 3
