Injured Worker (IW) jumped from the low end of a conveyor, landing on uneven ground and rocks. Per IW, the injury was "self-inflicted." The conveyor was approximately five feet high at the location IW jumped from. The injury resulted in a left talus fracture.

Mostrando los 6The Injured Worker (IW) was stepping out of the cone screen box and slipped on the lower end of the "to waste" conveyor frame. IW landed on feet but felt a pop in right knee and could not bear weight on it.
The Miner was removing a V belt guard on a JCI screening plant. Employee's foot slipped from where employee was standing, when employee's foot slipped employee fell forward into the side of the screen box resulting in a small cut to employee's lower lip area and a broken tooth.
Employee was getting down from working on equipment, slipped off ladder and landed on ground.
