EE had just finished installing new drive belts on feeder on 08IC01. He was sitting on a angle iron support frame handing tools to another employee when he slipped off the angle iron striking a frame support tubing with his left knee. Employee was helped out and transported by the supt. To Speaefish Regional Hospital where he was treated and released.
Injury was climbing up into the loader and hit the ladder with his left knee. He did not report the injury to us as a workplace injury until 05/26/09. This is a questionable work comp case because of conflicting information to the doctor and employer so is under investigation. I will update you when the case is denied or confirmed as a workplace injury.
Climbing off loader slipped and fell off and injured right arm
The employee was stepping off truck on uneven ground after loading and felt a sharp pain in the lower abdominal area. He went to the doctor and was diagnosed with a pulled abdominal muscle. He was released to full duty on 08/30/06. Employee was released from medical on 08/29/06. This employee does not work in the pit.
Walking on catwalk, slipped and twisted ankle.
Doing maintenance, fell off wash plant & landed on left arm and left leg.
