While employee was working in the quarry, he tripped and twisted his knee.
Employee was standing next to a wire trough that sits approximately 4" from the ground. As he was turning to walk, he caught his foot under the trough. This caused him to fall to the concrete.
While installing conduit, a fabricator fell approximately 6ft when the board he was standing on shifted. While the fabricator initially reported no injury, the next day he felt soreness in his back and was sent to the occupational health clinic. He was found to have severely pulled some muscles in his lower back. He was subsequently placed on light duty and returned to work.
EE was climbing a ladder, slipped and fell to the concrete.
EE was standing on catwalk of 350 Excavator hooking up battery charger to the excavator. Battery charger started to slip off catwalk and while EE tried to reach for the charger, he slipped and fell off catwalk to the ground.
Employee jumped up on passenger side of the truck to clean the window, he missed the step, fell back and when he landed on the ground, he twisted his left knee.
Walking from office to the loader, slipped on ice and fell. Fell on right leg and hip, hit head. Injury is described as Spinal Concussion.
