Employee step down on two steps stair and twisted his ankle then fell to the ground. The ankle swelled and, so employee left to go home in Sparks Nevada. Employee went to a health clinic and the ankle is fracture. Employee has not missed any work days as he is an administrator and will be working from an office to do his work.
EE is a dump truck driver/owner operator working thru T407. As he was on standby work due to a blasting operation, he was staged outside the pit area and in the North/East area of the quarry. While standing outside of his truck on a pile of material, he proceeded to step off the pile and lost his footing, thereby falling onto the ground.
Injury occurred while pile butt was repairing handrail at the Quarry spring line. He cut the handrail away and lost footing resulting in a fall to the water below, where he struck a float in the water. This resulted in two broken ribs and a punctured lung. Employee will be off duty until he has rehabilitated. Lost time injury.

HE SLIPPED AND FELL WHILE WALKING ON THE LOADING DOCK. HE STRUCK HIS LEFT HAND ON A METAL GRATING CAUSING A LACERATION. PROVIDED GLOVES BUT WAS NOT WEARING THEM.
CLIMBING DOWN FROM STRUCTURE, ROLLED LEFT ANKLE OUTWARD.
EE WAS RETURING FROM LUBE SHOP WALKING TOWARD TRUCK STOP. AT TOP OF KNOLL HE THEN STARTED DOWNSLIGHT DECLINE WHEN HE STEPPED IN A SLIGHT DEPRESSION. HE FELL FORWARD TWISTING HIS LEFT ANKLE
EMPLOYEE CLIMBED LADDER APROX 630 OF LOADER TO GO TO WORK AND DOWN ON OPPOSITE SIDE. BOTTOM STEP ON LOADER HAD BEEN KNOCKED OFF. EMPLOYEE DID NOT KNOW STEP WAS MISSING AND LOST BALANCE FALLING ON UPPER BACK AND SHOULDER. EMPLOYEE ONLY HAS ONE ARM WHICH DID NOT ALLOW HIM TO RECOVER HIS BALANCE.
WELDER HAD REMO5ED SAFETY CHAIN TO SECURE HIS WELDING LEADS I.NAME WALKED UP WALKWAY RAMP PAST CHAIN AND STEPPED THRU ACCESS OPENING FALLING TO THE GROUND
