We had snow/icy conditions. Preventive measures had been taken in the form of ice melt and sand prior to the injury. Employee slipped and feet came out from under her, falling backwards, hit her head. She walked inside and co-worker asked her what had happened- went for help. Then she was taken by private vehicle to medical facility - diagnosed concussion.

EMPLOYEE WAS WALKING ON PARTIALLY DRIED NEAT CEMENT. HIS RIGHT FOOT SLIPPED, BUT HIS LEFT FOOT DID NOT. AS HE FELL, HIS LEFT ANKLE TWISTED INWARD RESULTING IN A FRACTURED FIBULA.
