We had snow/icy conditions. Preventive measures had been taken in the form of ice melt and sand prior to the injury. Employee slipped and feet came out from under her, falling backwards, hit her head. She walked inside and co-worker asked her what had happened- went for help. Then she was taken by private vehicle to medical facility - diagnosed concussion.

Fell from a man-basket fracturing his left leg.

While getting on the crew bus employee tripped and fell. His right hand contacted the bottom step of the bus. This contact resulted in a fractured metacarpal bone behind his pinky finger. He did not seek medical attention until 11/16/2010.

Checked on employee when returning to office he slipped on ice.

While doing an pre-op inspection miner slip off loader and fractured foot.

While working on loader in engine compartment he slipped and contacted operating fan blade with hand.

After mixing mud, employee was coming down the stairs, set an empty bucket down on the lower platform, turned to walk down from the lower platform, slipped and fell to the ground on his shoulder.

EE WHO IS THE PERSON INVOLVED, WAS FUELING UP THE UNDERGROUND EQUIPMENT, AS HE WAS GOING AFTER ANOTHER PIECE OF EQUIPMENT HE SLIPPED ON THE FROZEN GROUND THAT HAD SOME RUTS IN IT, AND TWISTED HIS ANKLE. BREAKING THE TIBULA BONE IN HIS LOWERRIGHT LEG.

EMPLOYEE WAS WALKING ON PARTIALLY DRIED NEAT CEMENT. HIS RIGHT FOOT SLIPPED, BUT HIS LEFT FOOT DID NOT. AS HE FELL, HIS LEFT ANKLE TWISTED INWARD RESULTING IN A FRACTURED FIBULA.
