The miner was walking on a catwalk, tripped and tried to catch self injuring a shoulder as they fell down. Miner initially thought they were ok and did not report the accident to their supervisor until 4/27/2021. After the initial medical examination and a follow up, physical therapy was recommended.

It had rained and the ground was wet. The employee slipped and strained his lower back.
The employee was doing the walk around inspection of a haul truck when he slipped and turned his ankle causing an avulsion fracture on his left foot.
The employee was adding hydraulic oil to the dump truck when he slipped from where he was standing and fell. The fall resulted in him twisting his ankle. He thought it was a sprain, but when he went to the clinic to get checked they found his ankle was broken.
The employee was greasing the pins on his loader bucket and fell, striking his head.
Employee was making his rounds and tripped. He reached out to keep from falling down and his hand came in contact with the Number 4 kiln hood. He sustained a small 2nd degree burn on the side of his left hand.

ICE (BLACK), DARK AREA, UNEVEN GROUND. EMPLOYEE WAS WALKING DOWN A SLIGHT INCLINE, STEPPED ON SOME BLACK ICE, HIS FEET CAME OUT FROM HIM. HE FELL AND FRACTURED LEFT ANKLE. HE HAS SPLINT ON AND TUESDAY (12TH) WILL SEE AN ORTHOPEDIST TO DETERMINE IF SURGERY IS NEEDED.
EE WAS EXITING OUT OF BURNER SHACK TO RELAY MESSAGE TO ANOTHER EE WALKING ON STEEL GRATING DECK EE LEFT LEG WENT THRU AN OPENING IN DECK CAUSINGHIM TO GO DOWN, HIS RIGHT LEG BUCKLED UNDERNEATH HIM HE REAGGRAVATED THIS INJURY 2/3/01 WHILE USING A PICK WENT TO DR 2/15/01 FIRST VISIT
LUBRICATING EQUIPMENT FELL ON LEFT WRIST.
