Employee was responding to a fire at Screen Stone, he jumped out of the truck and grabbed two fire extinguishers, as he ran to the stairs he slipped in the mud and landed on his butt and back. Incident was not reported until 11/06/2014 because at the time he felt no injury.

Mostrando los 7Employee was sorting through a box of paperwork when a piece of paper fell on the ground the employee bent over to pick it up and his back went out causing him to fall forward and hit his head on the door. Employee was sent to the doctor for evaluation. No damage or impairment was caused.
Miner was stepping down off fork truck and twisted left ankle. There was no swelling and little pain at time. Miner completed shift. Over next few days, ankle continued to bother Miner. Visited doctor on 8/22 and 8/25/2008. Off work on 8/22 only. Diagnosed as sprain.

EE states, he was working on palletizer with other and slipped with 50lbs bag and his leg went one way, and he fell on left and broke his fall with knee on metal rail.
EE STATED, SHE WAS GOING TO HER CAR TO THE MILL AREA TO CLEAN, SHE WAS CARRYING SUPPLIES DOWN THE FRONT STEPS TO HER CAR WHEN SHE TWISTED HER RT. ANKLE AND FELL, BRUISING HER UPPER RT. ARM.
