Employee was exiting trackhoe. He smacked his hand on the track due to loss of footing because he was in a hurry and was not careful with his stategy of exiting the machine he was operating. Employee was in a hurry because the plant had shut down.

When exiting his loader one hand slipped off of the rail and spun his body around causing him to fall off of the ladder in which his hard hat fell off and he struck his head on a 55 gallon drum
Operator was climbing steps onto wheel loader. When on the third step going onto the fourth the operator said he was not watching what he was doing and slipped and fell onto his back on the ground.
EMPLOYEE WAS USING A FRONT END LOADER TO JUMP START A GENERATOR SET. STANDING ON THE LOADER ENTRY PLATFORM HE REACHED INTO THE CAB TO TILT THE SEAT FORWARD WHEN HIS FEET SLIPPED AND HE HIT HIS RIGHT SHIN AND KNEES ON THE THRESHOLD OF THE CAB DOOR. EMPLOYEE HAD MINER SCRAPE ON RIGHT SHIN. REFUSED ATTENTION AND RTW THAT MORNING. (STARTED MISSING 8-5-04 MAKING THIS REPORTABLE)
