EE was replacing head gaskets on 100 ton truck. After changing head gaskets, mechanic was in the process of bolting on the intake runners. To access the intake runners, the mechanic placed his left foot on the back motor mount. The mount still had the motor mount in place. The cap turned, causing the EE to fall approx 18". This caused a hairline fracture to the metatarsil bone.

EE was cleaning under the Pan Feeder at the Screen Plant. At break time he left to walk up to control room. As he walked down the 2 steps in this area, he stepped onto the expanded metal walkway. Walkway had small depression & he stepped in it, resulting in him twisting his ankle & falling down on walkway. Xrays were negative. Dx was ankle sprain & foot sprain.
EMPLOYEE STATES: WHILE CLIMBING DOWN FROM SERVICE TRUCK #TR02-025, HE STEPPED ONTO UNEVEN GROUND CAUSING HIM TO TWIST HIS ANKLE.
EE SHUT DOWN THE GENERATOR FOR THE END OF THE SHIFT. AS HE WALKED OUT OF THE GENERATOR TRAILER, HE STEPPED ON TO THE PLATFORM AT THE TOP OF THE STAIRS. THE PLATFORM WAS COVERED WITH ABOUT 1"OF SNOW. AS EE STEPPED ON THE SOLID METAL SECTION OF THE PLATFORM, HIS FEET WENT OUT FROM UNDER HIM AND HE FELL DOWN, STRUCK HIS ARM WHEN HE LANDED AND THEN HE SLID DOWN THE STAIRS.
CRUSHER WAS BEING REPAIRED, A PIECE OF METAL FELL FROM ITS RESTING PLACE AND STRUCK EMPLOYEE ON HARD HAT. THE BLOW UNBALANCED THE EE , HE FELL AND STRUCK HIS HEAD ON A STEEL FRAME, LACERATING HIS SCALP.
