While walking into the guard shack, the door was flung open, hitting the employee in the left arm, causing employee to fall back, jarring the shoulder and neck.
Employee was stepping down (approximately 2ft) off of the #2 Belt Head, he twisted his left ankle and leg.
Employee was standing beside of the roof bolter, bolting roof. The roof bolter was placed in reverse, employee stepped and stumbled over a small pile of coal and fell to his knees jarring his lower back.
The scoop man had knocked the stopping down. He was backing the scoop up so that he and another ee could pick up the loose blocks. EE was walking toward him. The scoop man looked away and when he looked back toward ee light going down as if he had stumbled and fell. EE was transported by ambulance to the hospital after he stumbled and fell
Going to check the stuck roller behind the head drive. Lost footing and fell.
EE was hanging a fly pad in the face of the section. He stepped in a hole and twisted his lower back. EE did not work on September 11, 2010. He did not work on September 13th, 14th 15th. He did work on September 16th, 17th, 18th, 19th & 20th. He did not again till October 4th, 5th, 6th. On Oct 6th he worked 6 1/2 hours. He has not worked since Oct 6th to date.

Contusions to shoulder and forearm worked 14th-15th pain worse swollen not on shoulder wnt to dr. Hospital refer him to dr. on 20th. He is to do MRI on 9-21-06.
The employee was on a 4' step ladder. When he went to get more bobond he slipped off of the ladder & landed on the mine floor.
