On 11/3/15 the mine operator was informed that the employee could no longer work at full duty due to a partial tear of the ACL in the left knee. On 10/22/15 the employee jumped from a ladder as the ladder was struck by a 28-inch ventilation tube elbow which was pushed into the ladder by a rib roll.

On 6/6/14 the employee could not return to work due to a right shoulder injury that occurred on 5/2/14. The employee stated that as he was attempting to hang a ventilation tube tee the loose coal that he was standing on shifted and he lost his balance straining his shoulder.

The injured employee is employed by Colorado Lime Company and was injured at Bowie Resources LLC, Bowie No. 2 Mine with MSHA ID 0504591. The injured employee slipped on an icy patch covered by snow and pulled his left hamstring. The treating physician placed him off work. He has returned to work with a full release.

The employee was found laying on the mine floor and was unresponsive to crew members. No cause for the employee falling could be determined. The employee may have a medical. The employee appears to have concussion like symptoms. The employee has a full duty release 1/27/14. Further medical testing is indicated.

While arriving for work the employee attempted to walk down the walkway from the parking lot to the bathhouse. Slick soled shoes caused the employee to slip and fall.

The employee stated that while packing a roll of curtain down the #4 lower b slope he slipped on the floor and fell. The employee stated that he suffers back disk degeneration.

While attempting to hang a length of water pipe from the mine roof the ladder used by the employee tipped over. The employee fell and landed on the ladder.

Standing on large rock on side of road. Lost footing and fell to the ground, resulting in dislocated elbow.

On 3/21/12 the employee underwent surgery for a torn ACL that appears to have occurred on 1/12/12 when the employee stepped down off of an idle conveyor belt to the mine floor and twisted his knee

EE and co-worker were violating company policy and partaking in horseplay during lunch. They were attempting to pour food condiments on each other, tripped and fell down together. Co-worker was on top of EE when they landed, injuring EE's shoulder.

While walking from parking lot to bath house upon arriving to work individual stepped down off of a 16 inch concrete ledge into loose gravel, slipping and then falling twisting his left knee.

On 5/19/10, the employee reported that he required shoulder surgery for the injury that occurred on 4/5/10. The employee stood on the 8-bay controller to unhang a hose from the roof. The employee fell off of the controller.

Two employees team lifted an 8-inch pipe valve to carry it. The injured employee stepped into a mine floor hole and sprained his heel ligaments.

On 9/15/09 the employee reported that he could no longer work on his ankle that he had twisted by stepping into a water hole on 8/26/09.

The employee fell from a ladder while avoiding a loose roof rock and struck his head on a pump.

Driver was attempting to get back in his truck and stepped on guard rail on scales. His foot slipped off and he fell to the ground injuring ribs.

On 4/29/09 the employee reported that a knee injury on 4/12/09 would require surgery to repair knee damage.

On 5/17/09 the employee left work with back spasms and back pain that has been attributed to the injury of 3/22/09. The employee tripped on a wheel chock rope for the pickup truck as he walked away from the truck.

Employee stated that he slipped stepping into ramcar and twisted his left ankle.

Was shoveling spillage around belt line stepping wrong and twisting his knee.

EE was unloading trash from back of p/u truck into dumpster. Lost balance falling from back of truck onto ground.

The employee traveled over construction material. When he slipped his arm was cut on the edge of a steel plate.

Slipped off miner and injured hip.

The employee fell from an unstable ladder and tore ligaments in ankle. Employee is currently off work.

The employee was carrying a conveyor belt idler to the scrap metal bin when he stepped on a patch of ice. The employee fell fracturing his left ankle.

Employee was standing on tracks of 992 excavator talking to supervisor. The tracks were muddy and when he moved to grab the handrail to climb down, he slipped off. He struck his left knee on running board, causing swelling, brusing and possible ligiment damage.
