Employee was injured when EE fell through an elevated platform in a restricted area of the underground mine after EE climbed over a berm to access the restricted, inactive area in violation of the mine's rules and EE's training. The employee was not authorized to enter the restricted, inactive area and was not performing any work activity at the time the accident occurred.

Original incident was reported to company on 4/16/2010 but not MSHA reportable/lost-time until 9/14/2010, thus the submission of the report. Incident occurred when climbing down from ladder when his foot/hand slipped off of handrail. Employee caught himself on safety chain on separate piece of equipment parked next to loader.
While entering office trailer, employee slipped, tripped and fell on steps leading into office trailer.
