Repairing a hydraulic leak on the 1200 Hitachi excavator. EE was on the second rung of the ladder. When EE broke loose a bolt, EE lost footing, which led to EE falling off the ladder to the ground. The ground surface was uneven where EE landed.

Mostrando los 19Employee was starting a P-70 pump when the line pressurized the line kicked the employees feet out from under him causing him to fall. The employee tried to catch himself with his left hand which caused him to fracture his left wrist. Due to no orthopedic doctor on duty the employee was referred to his orthopedic for medical treatment.
While walking quickly to turn off the remote switch the employee tripped and fell on the moving belt which caused his left leg to get caught between the belt structure. This caused a fracture of his left foot, fracture of his third toe and a laceration on his fourth toe. This is to report medical treatment only no days were missed.
After loading wire rope into a scoop bucket the employee turned and felt a sharp pain in his left knee. The employee was later diagnosed with a sprain and remains off.
On March 26th a section boss was putting up a center line when he twisted his left knee. He continued to work until May 21st when he had surgery, the employee missed 5 days but is now on restricted duty.
Employee was walking across floor where old screen was taken out and some old bolts were sticking up. Employee caught his right foot between the old bolts this caused him to lose his balance twisting his ankle and making him fall. This resulted in a fracture of the right foot. He started off 1/31/12.
On 11/14/11 EE informed safety he had went to seek medical attention from an incident on 11/11/11.While walking down the belt line he ran his head into a brow causing him to lose his balance and fall into the belt structure. It was determined that the employee had a fractured rib. This is to report medical treatment only NO days were missed.
Employee turned to walk inby in the belt entry and tripped over a piece of belt structure causing him to fall striking his left eye brow and the bridge of his nose. This caused a laceration that required sutures. This is to report medical treatment only NO days were missed.
While carrying a 1/4" x 4'x8' piece of sheet metal across the 2nd floor in the breaker building the foreman's feet slipped in loose material on the floor. He fell backwards and struck the back of his head on a 4" pipe then stuck a 4" steel angle beam beside the wall before landing on the floor on his back.
Stepping off last rung of 980G, left leg buckled and knee popped. Shook off and could walk on it. Friday night moved left leg slightly and knee popped and couldn't bend knee.
EE was dismounting a dozer when his foot slipped in mud on the track. This caused EE's foot to fall between the track and the dozer frame, causing him to lose his balance and fall forward. This caused a fracture of the lower Fibula. This is to report medical treatment only, no days were missed.
On 9/23/09 EE was getting off of a 2-man battery ride when his foot slipped twisting his right knee. He continued to work until December 16th,2009 when he had Arthroscopic Surgery on his meniscus. He was released to return to work on full duty December 17th, 2009. No days were lost, this is to report medical treatment only.
Employee injured his right knee while descending a stairway near the pre-wet screen. He continued to work and started off work on 9/10/09.
Setting left lift jack in place, strap slipped off knocking employee back causing him to fall cutting right forearm. Jack fell on left shoulder.

Utility man was walking from parking lot to the supply yard storage trailer when he slipped on the gravel and twisted his left ankle. He continued to work until 8/7/08 when he was diagnosed with a fractured metatarsal bone in his left foot.
While cutting a piece of belt to make a splice, the utility knife blade came out of the knife, causing EE to fall forward. When EE attempted to catch himself, he accidentally fell forward on the knife blade that was still stuck in the belt, lacerating his left hand which required several sutures. An employee of Superior Steam Cleaning & Sandblasting was on a small section of roof. He was removing sheeting on the side of East Gulf Prep Plant. As he was tying off his safety harness he slipped and fell though a skylight. He fell and cut his head on the right side.
While crossing the belt at the #2 tailpiece employee's right foot slipped off the tailpiece causing him to twist his right knee. He continued to work until the day of his surgery.
